DASH膳食模式联合低钠摄入水平可降低亚临床心脏损伤的两种标志物
心血管疾病(CVD)是全球的首要死亡原因。高钠摄入量和低水果蔬菜摄入量均可增加CVD风险。DASH膳食(Dietary Approaches to Stop Hypertension,终止高血压膳食疗法)已被确定对心脏生物标志物有有益影响。但减少钠的摄入量(单独或与DASH饮食结合)对心脏生物标志物的影响尚不清楚。
该研究旨在评估减少钠的摄入量与DASH膳食单用或联合应用对心肌损伤、牵拉和炎症的生物标志物的影响。
DASH-钠是一项对照喂养研究,研究对象为收缩压(BP)为120-159 mmHg、舒张压为80-95 mmHg的成年人,随机分至DASH膳食组和对照组。在其指定的饮食中,受试者连续4周摄入三种含钠水平(50、100和150 mmol/天)的食物。体重保持不变。日摄入能量为2100千卡。
共招募了412名受试者,平均年龄为48岁;56%的女性,56%的黑人。平均基线收缩压/舒张压为135/86 mmHg。
与对照组相比,DASH组的hs-cTnI降低了18%(95%CI: -27%至-7%),hs-CRP降低了13% (-24%至-1%),但NT-proBNP没有明显差异。相反,钠摄入量从高水平降至低水平,可降低NT-proBNP的水平,且不依赖于饮食(19%;95% CI: -24%至-14%),但不影响hs-cTnI的水平,而且hs-CRP 水平还轻度升高(9%;95%CI:0.4%至18%)。
与高钠-对照膳食相比,低钠-DASH膳食可显著降低hs-cTnI(20%;95%CI:-31%至-7%)和NT-proBNP(23%;-32%至-12%),但hs-CRP无明显改变(-7%;-22%至9%)。
总之,DASH膳食模式联合低钠摄入水平可降低亚临床心脏损伤的两种标志物。
