Radiology:年龄对脑损伤后患者工作记忆和脑功能活动的影响

目的:评估中度创伤性脑损伤(MTBI)后,年龄对工作记忆和脑功能活性的影响。方法:此研究得到当地研究伦理会批准。所有受试者签署知情同意书。运用功能磁共振(MRI)评估13名年轻MTBI患者(平均年龄26.2 岁 ± 2.9;范围在21–30 岁),13名老年MTBI患者(平均年龄57.8 岁 ± 6.6;范围在51–68 岁),26名年龄和性别配对的对照组的工作记忆和脑功能活性。获得损伤后1月,研

目的:评估中度创伤性脑损伤(MTBI)后,年龄对工作记忆和脑功能活性的影响。

方法:此研究得到当地研究伦理会批准。所有受试者均有签署知情同意书。运用功能磁共振(MRI)评估13名年轻MTBI患者(平均年龄26.2 岁 ± 2.9;范围在21–30 岁),13名老年MTBI患者(平均年龄57.8 岁 ± 6.6;范围在51–68 岁),和26名年龄和性别配对的对照组的工作记忆和脑功能活性。受试者的2次MRI影像学结果分别来自受伤后1个月时和初次研究后6周。在两组间对患者脑震荡症状、神经心理学测试和工作记忆活性进行组间比较和回归分析。

结果:与年轻对照组相比,年轻患者,在右侧楔前叶和右顶叶下回存在高反应性;然而与老年对照组相比,老年患者,低反应性存在于右侧楔前叶、右侧额叶下回。工作记忆活性增加与右侧楔前叶和右侧额下回脑震荡症状增加相关;工作记忆不良存在于年轻患者右侧楔前叶,而在老年患者中并不存在。在接下来的检测中,在年轻患者发现功能活性的部分恢复,脑震荡症状缓解,而在老年患者中并未发现此现象。

结论:老年和年轻患者功能MR影像显示脑震荡症状呈现不同表现,证明年龄在MTBI后对患者脑功能活性、调节以及工作记忆起着重要的作用。这些结果也表明年轻患者比老年患者有着更好的神经可塑性和临床恢复能力。
