Circulation:低血钾症可进一步引发心律失常

低钾血症对心室颤动具有一定程度的不利影响。研究人员推测蜂毒明肽敏感的小电导钙激活钾通道电流(/KAS) 在低血钾症期间被激活并且/KAS的阻断导致了心律失常的发生。研究对23例伴有房室传导阻滞和在血钾正常或低钾血症时左或右心室能够发生起膊的离体兔心室进行光学映像检测。结果显示,蜂毒明肽延长了动作电位持续时间 (APD),以1000毫秒作为起膊周期,在血钾正常情况下,26毫秒期间(95% 置信区间

研究对23例伴有房室传导阻滞,和在血钾正常或低钾血症时,左心室或右心室能够发生起膊的离体兔心室进行光学映像检测。结果显示,蜂毒明肽延长了动作电位持续时间 (APD),以1000毫秒作为起膊周期,在血钾正常情况下,26毫秒期间(95% 置信区间 [CI], 14–37)达到80%的复极化状态(APD80),而在低钾血症情况下(P=0.01),达到相同状态需要54毫秒的时间(95% CI, 40–68)。在低钾性心室中,蜂毒明肽增加了APD恢复的最大斜率,APD交替的起膊周期长度阈值,波形破碎效应的起膊周期长度,以及非协调性APD交替区域的面积。蜂毒明肽能促进诱发持续性心颤(P=0.009)。短期心脏记忆由APD80的斜率与激活时间共同决定。在右心室起膊过程中,经过蜂鸣毒素的刺激(P<0.001),斜率从处在基线的0.01(95% CI, 0.09-0.12)上升至0.34(95% CI, 0.23–0.44),而在左心室起膊过程中,经过蜂鸣毒素的刺激(P=0.045),斜率从0.07(95% CI,0.05 - 0.20) 上升至0.54(95% CI, 0.06–1.03)。通过膜片钳技术进行研究后,研究人员证实在低钾血症期间检测到IKAS在隔离兔心室中的量有所增加。

低钾血症通过激活IKAS从而缩短APD并且在心室起膊过程中维持了后期激活位点的复极储备。IKAS阻断显著延长了后期激活位点的APD并且促进了心室颤动效应。
