Blood:巨噬细胞低吞噬作用介导mAb诱导性细胞清除的免疫耐受

由于表面的FcRs减少,巨噬细胞的吞噬作用持续减弱(hypopophagia[即巨噬细胞的吞噬作用低]);巨噬细胞是多种治疗性mAb的关键效应细胞,hypophagia可能会导致患者对mAb耐药。

巨噬细胞抗体依赖性的细胞吞噬作用(ADCP)是治疗性单克隆抗体(mAb,如利妥昔单抗)和抗体诱导的溶血性贫血和免疫血小板减少的主要细胞毒性机制。

近期,《Blood》上发表了一篇文章,Pinney等人研究了对调控巨噬细胞在高靶效应细胞比率(如白血病患者的循环肿瘤负荷)情况下进行ADCP的速率和能力的机制。

通过对原发人类患者和小鼠的巨噬细胞进行定量活细胞成像,研究人员发现在最初受到mAb-调理淋巴细胞的刺激后,巨噬细胞会经历短暂的快速吞噬爆发(<1小时),随后吞噬活性急剧下降,可持续数天。ADCP的这个既往不明确的不应期,或“hypophagia”,在所有的巨噬细胞/mAb/靶细胞体外测试条件下均有出现,并在接受连续的αCD20 mAb依赖性的循环B细胞清除的小鼠的体内Kupffer细胞中也有观察到。

重要的是,hypophagia对抗体依赖性的吞噬没有影响,也不改变巨噬细胞的存活能力。在机制研究中,研究发现,巨噬细胞表面激活的Fc受体快速丢失以及随后发生了蛋白降解是hypophagia中ADCP活性丧失的主要机制。

这些研究结果表明hypophagia是通过ADCP进行的巨噬细胞依赖性的细胞清除的关键的限制步骤,了解这类对固有免疫细胞的细胞毒性能力的限制有助于开发mAb疗法,或可进一步优化ADCP,并改善患者预后。
