CLIN CHEM LAB MED:测定血清醛固酮三种不同的化学发光测定方法和快速液相色谱串联质谱法的比较

20176月至8月在北京协和医院,研究人员对221例疑似原发性醛固酮增多症患者的血清进行回顾性分析。采用LC-MS/MS方法和三种不同的CLIA系统(: DiaSorin®XLiSYSAuto Lumo A2000)检测sALD水平,并使用混合新鲜血清样本用于重新校准。使用Passing-Bablok回归分析、相关矩阵和Bland-Altman图来评估使用三种CLIAs测定的ALD水平之间的一致性。

研究结果显示,除一种CLIA的系数变化(CV)对于低sALD水平的样品超过20.0%之外,四种测定的实验室内精密度在2.1%至9.4%范围内。 使用LC-MS / MS测定的sALD水平显着低于使用其他三种CLIA测定的sALD水平(p <0.0001)。 四种测定的Spearman相关系数在0.7450.950之间(p <0.0001)。 Bland-Altman图显示,三种CLIALC-MS / MS的平均偏差(%)在-69.3-49.2之间。 重新校准后,这些相关性在测定之间没有得到改善。 但是,在LC-MS / MSDiaSorin®XL / iSYS之间,医疗决策级别的偏差和偏差百分比有所提高。

研究结果表明,CLIAsLC-MS/MS的结果之间存在显著的不一致性,这表明不同的sALD措施不能互换使用。
