  方法 我们在91例慢性淋巴细胞白血病患者中获取了来自白血病细胞的DNA样本,并且对88个全外显子组和全基因组进行大规模平行测序,以及对匹配的种系DNA进行测序,以鉴定这种疾病的体细胞突变谱。

  结果 我们辨别出了9个以显著频率突变的基因,包括4个在慢性淋巴细胞白血病中有确定作用的基因(15%的患者中的TP53,9%中的ATM,10%中的MYD88,4%中的NOTCH1)和5个作用尚不确定的基因(SF3B1,ZMYM3,MAPK1,FBXW7和DDX3X)。SF3B1(在剪接体催化核心发挥作用)是第二常见的突变基因(突变发生于15%的患者)。SF3B1突变主要发生在染色体11q缺失的肿瘤,这些突变与慢性淋巴细胞白血病患者预后差相关。我们进一步发现了在SF3B1有突变的肿瘤样本在前信使RNA(mRNA)剪接(过程)中有改变。

  结论 我们的研究确定了慢性淋巴细胞白血病体细胞突变的图景,并且强调了前mRNA剪接作为导致慢性淋巴细胞白血病的关键细胞过程。
