研究发现,甲状腺手术中使用血管封闭器与减少颈部血肿的几率有关,而不增加神经损伤的几率
血管封闭器(VSDs)已被广泛应用于甲状腺手术中,近日研究人员考察了甲状腺手术中VSD,常规止血(CH)与颈部血肿形成的关系。
本回顾性队列研究从2016年1月1日至2017年12月31日评估了甲状腺手术--国家外科质量改善计划目标数据库中的10903名患者。研究的主要结局为术后血肿,需要进行开放式排空干预,二次手术、气管造口术、观察或延长住院时间。次要结局包括喉返神经损伤、手术持续时间和住院时间。
6522名患者,平均年龄52岁;8544名女性。与VSD相比,CH与颈部血肿风险增加相关(比值比,2.33),VSD组中有34例(1.0%)血肿,而CH组中78例(2.4%)血肿。次要结局中,与VSD组相比,CH组住院时间较长(发生率比:1.29),但喉返神经损伤发生率(比值比:0.90)或手术持续时间(发生率比:0.99)无显著差异。
