JNNP:基于言语和语言评估的全自动认知筛选工具

记忆门诊评估各种认知障碍患者,包括与阿尔茨海默病(AD)、轻度认知障碍(MCI)和功能性认知障碍(FCD)相关的患者。早期转诊到适当的护理途径有利于患者的健康和医疗资源的有效分配。2009年至2014年间,记忆门诊转诊增加了682%,这表明需要准确的临床前筛选工具,能够引导患者选择最合适的服务。AD与语言的细微损伤有关,这种损伤可能先于情景记忆的缺失长达数十年。先前的研究表明,受会话分析方法(CA)启发,对会话特征进行定性分析可以区分FCD患者和神经退行性疾病(ND)患者。然而,这种方法依赖于训练有素的专家,而且不易扩展。虽然使用自动语音分析来识别认知障碍已经被研究过,但大多数研究并没有描述完全自动化的解决方案。他们依赖于从人与人之间的互动中收集的数据进行自动分析,或者使用手动生成的转录。本文创造了一个完全自动化的分层工具。“CognoSpeak”由一个虚拟的临床医生组成,一个电脑屏幕上显示的说话头,可以询问问题并记录患者的口头反应。该系统使用自动语音识别(ASR)和日记(将录音分成不同说话人的稿件)来提取声学和语言度量,使用机器学习分类器来选择最可能的诊断类别。

60名参与者;四个不同的诊断组各有15名:AD、MCI、FCD和健康对照组(HC)。患者参与者来自谢菲尔德的一家专业记忆诊所。病人可以自己或在陪同人员在场的情况下进行记录。神经系统的诊断是根据标准诊断标准和经过多学科小组审查后作出的。存在显著的情绪障碍(通过临床病史和患者健康问卷(PHQ-9>15)确定)和显著的脑血管疾病导致排除。所有受试者均采用Addenbrooke的认知检查修订(ACE-R)工具或详细的神经心理学评估进行认知评估。根据临床需要进行脑显像(包括CT、MRI和Tc99m-HMPAO单光子发射CT)。健康对照组的一部分人在之前的研究中做了核磁共振检查(VPH-DARE@IT公司http://www.vph-dare.eu/). 所有参与者都是以英语为母语的人,大多数是英国白人。虽然少数参与者来自南亚裔,但都是以英语为母语或双语的人。使用ASR系统将音频转录成一串单词。自动转录的文本和录制的音频被用于提取基于机器学习的分类器(logistic回归)用于将参与者分配到诊断类的一系列特征。从患者和随行人员的语音中提取72个特征,包括17个CA启发特征、24个纯声学特征、24个词汇特征和7个词向量特征。

所有参与者组和认知障碍患者的互动平均持续时间为11分钟24秒。HCs平均花费9分钟58秒;FCD患者11分钟2秒;MCI患者9分钟34秒;AD患者15分钟2秒。只有MCI组和AD组之间的差异显著(U=59,p=0.026)。在可能患有神经退行性疾病和非进行性疾病的患者之间的双向区分中,Cognospiak系统识别MCI或AD的准确率为87%,而将参与者正确分配为HC或FCD的准确率为77%。这相当于神经退行性记忆障碍的敏感性为86.7%,特异性为76.7%。AD,阿尔茨海默病;FCD,功能认知障碍;HC,健康对照。65%的病例分类正确。识别AD患者的准确率为80%。两名参与者被错误地分配为MCI,另一名被分配为FCD和HC组。MCI的识别准确率为80%。一名参与者被错误地分配为AD,两名参与者被错误地分配为FCD和HC。对HC或FCD患者的识别准确率为50%。8名参与者被错误地分配为MCI,7名被错误地分配为AD。这相当于80.0%的敏感性和77.8%的特异性。在四个小组中,60%的被试在整个任务中的识别准确率更高。识别AD参与者的准确率为80%;识别MCI参与者的准确率为60%;识别FCD参与者的准确率为47%,识别HC参与者的准确率为53%。

与最常用的筛选工具相比,双向分类系统的灵敏度达到86.7%。特异性略低,为76.7%,但考虑到早期识别的重点以及“假阳性”将由专业记忆服务机构进行调查的事实,这是可以接受的。使用三向分类法,CognoSpeak能够识别MCI患者,其敏感性为80.0%,特异性为77.8%,超过MMSE(mini-mentalstate examination,MMSE)在区分MCI和HCs时的敏感性(66.34%)和特异性(72.94%)。对MCI患者进行准确的鉴定,有助于早期干预,并有助于参与研究。还不知道有任何其他的筛查工具将FCD患者纳入他们的研究。在筛选层工具的验证研究中,纳入该患者群体是必不可少的,因为FCD患者占专家记忆服务转诊的24%。本文的工具能够区分FCD和MCI/AD,而不仅仅是HC和MCI/AD组,这增加了它的有效性。未来的研究还将包括非AD痴呆和老年痴呆的患者群体。
