研究人员将SAO的光镜诊断与标准PCR基因分型进行了比较。3名训练有素的显微镜专家分别对971Giemsa染色的薄血膜进行了评估,经PCR确认了SAO基因型。采用广义混合模型估计光镜与“金标准”PCR的敏感性、特异性、阳性预测值和阴性预测值。

结果显示,在评估的红细胞形态学参数中,刀状细胞是SAO状态的最强预测因子,而不是卵形细胞形态学(odds ratio [OR] = 19.2;95%置信区间[95% CI] = 14.625.3;P0.0001)。以刀细胞为中心的显微方法的诊断性能依赖于显微镜工作者:两名显微镜工作者应用该方法的灵敏度为0.89,特异性为0.93。显微镜镜检人员(κ= 0.20)以及显微镜工作者和金标准之间(κ= 0.36)表现不佳,主要是由于一名显微镜人员将气孔细胞误分类为刀状细胞,但在排除容易出错的读取器后,性能得到了显著改善(分别κ= 0.650.74)

研究表明,刀状细胞提示对SAO的光镜诊断可与耗时和昂贵的分子方法相媲美,但此方法需要特殊的训练,包括需要将刀状细胞从口型细胞中成功地将区分出来。
