Diabetes Care:血栓切除术后急性大血管闭塞性卒中合并糖尿病患者卒中前血糖控制与功能结局的关系

卒中前血糖控制目标HbA1c≤7.0%可能有益于接受IAT治疗的大血管闭塞性卒中合并糖尿病患者的神经功能恢复。
近日,糖尿病领域权威杂志Diabetes Care上发表了一篇研究文章,该研究旨在评估卒中前血糖控制是否与接受动脉内血栓切除术(IAT)的急性大血管闭塞性卒中合并糖尿病患者的功能结局相关。
研究人员从来自韩国卒中临床研究中心注册中心纳入了2009年1月至2020年3月期间接受IAT的紧急大血管闭塞性卒中合并糖尿病患者。研究人员评估了入院时HbA1c水平与功能结局(卒中后3个月的改良Rankin量表)之间的关联。
该研究共分析了1351名患者。与HbA1c水平较低的患者相比,入院时HbA1c水平较高的患者早期神经功能恶化更常见(根据HbA1c五分位数,P=0.02,根据HbA1c为7.0%的临界值,P=0.003)。
入院时较高的HbA1c水平与良好功能结局的几率降低显著相关。在根据年龄、性别、卒中亚型、闭塞部位、再通程度、溶栓方式、从症状出现到腹股沟穿刺时间和治疗期划分的亚组中均一致地观察到了这种关联。
由此可见,卒中前血糖控制目标HbA1c≤7.0%可能有益于接受IAT治疗的大血管闭塞性卒中合并糖尿病患者的神经功能恢复,无论卒中亚型、静脉溶栓、闭塞部位、再通程度和治疗周期如何。
