Int J Cardiol:参考范围内较低的游离三碘甲腺原氨酸水平与较高心血管死亡率相关

由此可见,参考范围内较低水平的FT3可以独立地预测一般人群中较高的心血管死亡率。

甲状腺激素在血管稳态中起着重要作用。较低的游离三碘甲腺原氨酸(FT3)水平与几种疾病较差的预后相关。然而,与促甲状腺激素(TSH)和游离甲状腺素(FT4)相反,FT3在一般人群中的发病率和死亡率中的作用仍然不确定。近日,血管领域权威杂志International Journal of Cardiology上发表了一篇研究文章,研究人员旨在评估正常范围FT3水平与一般人群死亡率之间的关联。

研究人员对2001-2002、2007-2008和2009-2010年间的全国健康和营养检查调查(NHANES)中7116名成年人进行了评估,并计算了截至2011年12月的死亡率。排除标准为:妊娠、甲状腺疾病史、使用甲状腺相关药物以及TSH、FT4或FT3水平超出参考范围。

在45个月的中位随访期间,357名参与者死亡。在未经调整的分析中,较低的FT3水平与更高的全因(FT3每0.1pg/mL增加的HR:0.82 [95%置信区间为0.78-0.87])、心血管(HR为0.74 [0.66-0.83])、癌症相关相关(HR为0.88 [0.80-0.97])和其他与病因相关的死亡率(HR为0.83 [0.77-0.90])。在使用Cox比例风险模型进行调整后,较低的FT3水平仍然与较高的心血管死亡率(HR为0.83 [0.75-0.93])显著相关,但与全因(HR为0.97 [0.92-1.02])、癌症相关(HR为1.02[0.89-1.17])或其他病因相关的死亡率(HR为1.00 [0.92-1.10])无关。 

由此可见,参考范围内较低水平的FT3可以独立地预测一般人群中较高的心血管死亡率。

甲状腺激素测定包括TT4、TT3、FT4、FT3及TSH。血中的T4、T3 中99%以上与蛋白结合,进入细胞内发挥作用的占总激素不到1%的游离激素。怀孕、肝功能损害等导致结合蛋白浓度改变对总激素浓度测定产生很大影响,而游离激素的浓度基本不受影响,因此游离激素可判断甲状腺激素过多、不足。
