Stroke:溶栓后不明原因的早期神经功能恶化与血栓扩展有关?

溶栓后不明原因的早期神经功能恶化与磁敏感血管征扩展独立相关,提示原位血栓延长或再栓塞。这些研究结果表明对溶栓治疗后早期抗血栓治疗来降低不良临床事件的风险的进一步研究很有必要。

前循环卒中后早期神经功能恶化与预后不良密切相关。除了简单直接的原因外,如脑出血和恶性水肿。在大多数情况下,静脉溶栓后发生早期神经功能恶化的机制仍不清楚。近日,卒中领域权威杂志Stroke上发表了一篇研究文章,研究对不明原因的早期神经功能恶化与血栓扩展有关的这一假设进行了检验。

研究人员从连续进行溶栓治疗的患者数据库中挑选了前循环卒中患者,并且这些患者均在入院时和发病后24小时进行了T2磁共振成像检查,在入院时进行磁共振血管造影可见血管闭塞部位,而且在发病后24小时磁共振血管造影显示闭塞部位仍没有开通。早期神经功能恶化被定义为24小时临床评估NIH卒中量表恶化评分≥4,不明原因的早期神经功能恶化被定义为没有明确原因的早期神经功能恶化。在T2成像磁敏感血管征发生扩展的定义为从入院到24小时后续磁共振中任何新出现的或者扩展的磁敏感血管征,并在不明原因的早期神经功能恶化和没有早期神经功能恶化的患者对这项指标进行了比较。

在这项研究中有120名符合条件的患者,22名参与者出现了不明原因的早期神经功能恶化。14名(34%)参与者存在磁敏感血管征扩展,相比于无不明原因的早期神经功能恶化的患者,在不明原因的早期神经功能恶化的患者中磁敏感血管征扩展更加频繁(分别为59% vs. 29%,调整后的比值比为3.96;95%可信区间为1.25-12.53;P=0.02)。

在本研究中,溶栓后不明原因的早期神经功能恶化与磁敏感血管征扩展独立相关,提示原位血栓延长或再栓塞。这些研究结果表明对溶栓治疗后早期抗血栓治疗来降低不良临床事件的风险的进一步研究很有必要。
