J Rheumatol:高血压风险升高与脊椎关节炎病程相关

ASAS-COMOSPA队列中,SpA病程与患HTN的几率较高相关,尤其是中轴型患者。

脊柱关节炎(SpA)与许多血管(CV)合并症有关。这项研究探讨了SpA病程与CV相关疾病延误诊断之间的关联。

使用来自COMOSPA研究的数据,在单因素和多因素logistic回归模型中评估了SpA病程与CV相关疾病之间的关联。每个模型分析1个CV相关因素作为因变量,"SpA病程"作为预测因子,并根据相关混杂因素进行调整。

3923名受试者的数据可用于分析(SpA中位病程为5.1年,四分位范围为1.3-11.8年)。主要的CV相关疾病是高血压(HTN; 22.4%)、缺血性心脏病(2.6%)、卒中(1.3%)和糖尿病(5.5%)。在单因素和多因素分析中,HTN与SpA病程相关,SpA病程每增加5年,OR为1.129(95%CI 1.072-1.189; p<0.001)。与HTN相关的其他因素是年龄、男性、当前的体重指数、曾经类固醇治疗和合成疾病改善性抗风湿药治疗,但不是非甾体类抗炎药(NSAID)。亚组分析中,仅中轴受累的SpA表型受试者HTN与病程的相关性最强(OR 1.202,95%CI 1.053-1.372),而不是仅外周受累的患者(OR 0.902,95%CI 0.760-1.070)。其他CV疾病与SpA病程无关。

ASAS-COMOSPA队列中,SpA病程与患HTN的几率较高相关,尤其是中轴型患者,但SpA病程与其他CV相关疾病无关。与HTN的相关性似乎与使用NSAID无关。
