在手部骨关节炎患者中,较高的BMI与手、脚和膝盖/臀部的疼痛严重程度相关。以瘦素衡量的肥胖全身效应可能比下肢疼痛在手部疼痛中起更大的介导作用。
骨关节炎(OA)是最常见的以疼痛为主要症状的关节疾病。体重是OA疼痛潜在的可改变危险因素,体重减轻可能会降低膝关节OA患者的疼痛、机械负荷和全身炎症。由于肥胖引起的机械负荷对手关节的影响。
近日,风湿免疫学领域权威杂志Arthritis & Rheumatology上发表了一篇研究文章,该研究旨在明确体重指数(BMI)与手部骨关节炎患者疼痛之间的关系,并探讨这种关联是否由全身炎症生物标志物介导,如果是因果关系。
在281名Nor-Hand研究参与者中,研究人员评估了BMI与通过澳大利亚/加拿大手部骨关节炎指数(AUSCAN,范围0-20)和数值评定量表(NRS,范围为0-10)确定的手痛、通过NRS(范围为0-10)确定的足痛、西安大略/麦克马斯特大学骨关节炎指数(WOMAC,范围为0-20)的膝盖/臀部疼痛、疼痛的全身关节计数和疼痛敏感性之间的关联。研究人员拟合自然效应模型,评估了BMI与通过炎症生物标志物确定的疼痛的自然直接和间接影响。
BMI每增加5个单位与更严重的手部疼痛(AUSCAN平均增加0.64,95%CI为0.23-1.08)、足部疼痛(NRS平均增加0.65,95%CI为0.36-0.92),膝/髋疼痛(WOMAC平均增加1.31,95%CI为0.87-1.73),全身疼痛和致敏相关。介导分析表明,BMI对手部疼痛和全身关节疼痛计数的影响分别由瘦素和高敏C反应蛋白(hs-CRP)部分介导。瘦素介导的效应量在手部大于在下肢,并且仅在手部具有统计学意义。
由此可见,在手部骨关节炎患者中,较高的BMI与手、脚和膝盖/臀部的疼痛严重程度相关。以瘦素衡量的肥胖全身效应可能比下肢疼痛在手部疼痛中起更大的介导作用。由hs-CRP反应的低度炎症可能导致超重/肥胖个体的全身疼痛。
