用药错误是指合格药品在临床使用全过程中出现的、任何可以防范的用药不当。为防止或减轻用药错误对患者的伤害,我国2014年发布了《中国用药错误管理专家共识》,简要介绍了用药错误的防范策略。2018年10月,美国卫生系统药师协会(ASHP)发布了《预防院内用药错误指南》,从11个方面详细介绍了医院用药错误的预防策略。本文对该指南进行简要解读,对药师在预防用药错误方面的作用和职责进行详细论述,为药师提供切实可行的建议和最佳实践。
