近期,来自美国的研究人员在《JAMA Otolaryngol Head Neck Surg》上发表文章,调查了信号敏感性和中枢听觉处理与痴呆及阿尔茨海默氏症(AD)风险之间的关联。

研究为随访观察设计,在华盛顿州的一家西部医疗服务系统中进行,患者来自一项前瞻性成人思维变化(ACT)痴呆风险研究。包括了280名没有痴呆症的志愿者参与者,并在2003年10月至2006年2月期间接受了评估,随访至2018年9月。从2019年开始进行数据分析,一直持续到2021年。

听力测试包括纯音信号可听性、单声道单词识别测试和两项双通道测试:双声道句子识别(DSI)测试和双声道数字测试(DDT)。每两年用认知能力筛查工具评估一次认知能力(范围1-100;分数越高越好),分数低于86分则需要进行临床和神经心理学评估。所有数据都均在多学科的共识会议上进行了审查,并使用标准化的准则来定义痴呆症和可能的AD病例。Cox比例危险模型用来确定与听力测试表现的关联。

研究最终包括了280名参与者(177名女性[63%];平均[SD]年龄,79.5[5.2]岁)。截至2018年9月,共有2196人年的随访(平均,7.8年)。研究中,共发生89例痴呆症事件(66例未进行过分析),其中84例(94.4%)为AD(63例未进行过分析)。与DSI评分超过80分的人相比,DSI评分低于50分的参与者患痴呆症的调整危险比(aHR)为4.18(95% CI,2.37-7.38;P<.001);DSI评分为50至80分的参与者危险比为1.82(95% CI,1.10-3.04;P=.02)。与DDT评分超过80分的人相比,DDT评分低于50分的痴呆aHR为2.66(95% CI,1.31-5.42;P=.01);DDT评分为50至80分的人为2.40(95% CI,1.45-3.98;P=0.001)。AD结果也类似。纯音均值与痴呆和AD的关系微弱且不明显,当控制DSI分数时,则没有关联。

综上所述,中央听觉处理异常与痴呆和AD风险独立相关
