背景:大量研究表明,多种药代动力学和遗传药理学因素都可对5-FU的疗效和毒性造成影响。这些因素包括系统药物暴露(浓度-时间曲线下面积,AUC)、胸苷酸合成酶(TS)和二氢嘧啶脱氢酶(DPD)。本研究的目的是探讨这些因素与5-FU联合顺铂在鼻咽癌患者中的疗效和毒性之间的相关性。

方法:122例具有可测量病灶的NPC患者被纳入研究,接受顺铂(80mg/m2,第1天)联合5-FU(4g/m2,civ120h)疗法治疗;在5-FU输注的稳定状态下,收集患者的血液样本;使用一免疫测定法(MyCare)对5-FU的AUC进行测量。基于5-FU相关的黏膜炎和治疗反应,使用接受者操作特性曲线(ROC)确定5-FU的AUC目标范围。使用免疫组织化学法(IHC)收集肿瘤组织,用于评估TS和DPD的表达。

结果:共108例患者接受了分析研究(5例不可评估,6例退出研究,3例为无效样本)。5FU的AUC值介于15至103mg•H/L之间。5-FU的AUC的最佳目标范围为31-37mg•H/L,20.4%的患者在目标范围内,而39.8%的患者在目标范围以下,39.8%的患者高于目标范围。与低于目标范围的患者相比,在目标范围内或高于目标范围的患者有较好的客观反应率(75.4%vs 55.8%,P = 0.033);与高于目标范围的患者相比,在目标范围内或以下的患者有较低的毒性反应率(3/4级)(6.2%vs 67.4%,P<0.001)。基于AUC以及DPD和TS的表达情况制定的、可预测治疗反应的诺模图模型表明,所有独立变量都在目标AUC范围内、DPD较高或TS较低的肿瘤患者有较好的治疗反应率。

结论:药代动力学和遗传药理学相结合的方法可能是优化晚期鼻咽癌患者化疗疗效的一种创新策略。本研究的这些发现应在更多的前瞻性研究中加以确认。
