Neurology:脑结构与功能作为淀粉样蛋白中间介质影响记忆

这是一个前瞻性的队列研究。我们测量了Aβ、脑部功能及构造的基线水平。通过介质分析,检测统计出的Aβ 阳性对横向及纵向情景记忆的影响是否以代谢减退或局部的灰质容积为介质。分析分为两组进行:认知健康对照组(CN, n = 280)及轻度认知功能障碍组(MCI, n = 463).。

研究结果:
较低的记忆力得分与Aβ 阳性、较小灰质容积及代谢减退相关。在MCI组,Aβ 的影响以较小容积及代谢减退为介质,在CN组则不然。最强的独立区域介导率约为25%,脑部构造及功能介导率约为40%。在若干区域,灰质萎缩和代谢减退可能会影响情景记忆,与Aβ 阳性无关 (at p < 0.05)。

研究结论:
某种程度上,脑部构造和功能的改变似乎是Aβ 病变的后阶段的结果,最终导致情景记忆的减退。然而,Aβ 病变也会通过不能用这些影像测量量化的机制与记忆力减退密切相关,且情景记忆力的衰退部分是由与老年痴呆症相似的脑部变化导致,与Aβ 病变无关。
