Thyroid:早期暴露于中国大饥荒与甲状腺功能和疾病的关系

胎儿期大饥荒暴露与成年期TSH水平升高有关。胎儿阶段可能是垂体-甲状腺轴发育的关键时期。

早年营养不良可能会永久改变身体的结构和功能,导致成年后出现多种疾病。生命早期暴露于饥荒对甲状腺功能和疾病的影响尚未明确。近日,甲状腺疾病领域权威杂志Thyroid上发表了一篇研究文章,这项研究调查了生命早期暴露于大饥荒(1959-1961)与甲状腺功能和疾病之间的关系。

该研究纳入了9881名受试者,这些受试者均来自甲状腺疾病、碘状况和糖尿病流行病学调查。甲状腺功能和疾病通过所有参与者的血液样本和超声检查结果进行确定。研究人员通过二元Logistic回归和线性回归评估了早期饥荒暴露与成人甲状腺功能和疾病之间的关系。

与未暴露于大饥荒的参与者相比,在胎儿期暴露于中国大饥荒的参与者成年后TSH水平更高(β=0.024;P=0.038)。在农村参与者中,这一关联显著(β=0.039;P=0.02),而在城市参与者中这一关联则不显著(β=0.005;P=0.77)。胎儿暴露组没有比年龄匹配的对照组具有更高的甲状腺疾病风险,包括明显的甲状腺功能亢进、亚临床甲状腺功能亢进、明显的甲状腺功能减退、亚临床甲状腺功能减退、自身免疫性甲状腺炎和甲状腺结节(P>0.05)。
