研究人员在同意参加两项前瞻性3期试验(FIRSTANA和PROSELICA)的可选子研究的mCRPC患者中获取了血浆lpWGS数据。在FIRSTANA中,未曾化疗的患者随机分配到多西他赛(75mg/m2)或卡巴他赛(20或25mg/m2)治疗。在PROSELICA中,之前接受过多西他赛治疗的患者随机分配到20或25毫克/平方米的卡巴他赛治疗。lpWGS数据来自188名mCRPC患者在四个不同时间点(筛选、第1周期、第4周期和研究结束)获得的540份样本。

在单变量和分层多变量分析中,血浆肿瘤分数对总生存期有预后作用(危险比1.75,95%置信区间1.08-2.85;P=0.024),与现有生物标志物相比,血浆肿瘤分数提供了附加值(C指数0.722 vs 0.709;P=0.021)。纵向变化与药物反应有关。PROSELICA样本富含LSTs(p=0.029),表明了基因组不稳定,这种富集与之前的阿比特龙和恩杂鲁胺治疗有关,但与他汀类或放疗无关。较高的LSTs与RB1/RNASEH2B的损失相关,与BRCA2的损失无关。

cfDNA肿瘤分数评估具有预后性

他们研究了前列腺癌患者血液样本中的肿瘤DNA,发现血液中的肿瘤DNA水平能够表明疾病的预后,而且可以检测到治疗后的变化。他们还观察到结果中的 "基因疤痕 "与之前的某些治疗有关。他们的测试允许对肿瘤活动进行评估,可以补充现有的测试来改善对药物反应的认识,并检测临床相关的基因变化。
