FDA建议中枢神经系统感染时进行多病原核酸检查

美国食品药物管理局(FDA)批准了第一个依赖核酸检测的脑脊液(CSF)检查,该检测方法能同时检测出14种引起中枢神经系统(CNS)感染的病原体。

BioFire Diagnostics公司的FilmArray Meningitis/Encephalitis (ME) Panel被设计用来检测同时表现出CNS感染的症状和体征的患者的脑脊液标本,约60分钟后得到测试结果。而 FDA新闻发布,应用现在的诊断技术,需要3天才能确定细菌感染。病毒感染的等待时间更长,因为检测人员必须转到专门的实验室。

FDA指出,FilmArrary ME Panel较快的周转可以使临床医生在早期作出较好的治疗决策。

FilmArrary ME Panel需要少量的CSF样本。FDA说,当前,检测多个体的CSF是不可能的,因为很难获得足够的脑脊液进行多种测试。

新的测试识别下列病原体:


1.细菌和酵母菌:大肠杆菌K1 ,流感嗜血杆菌,李斯特单核细胞增生菌,脑膜炎奈瑟菌,无乳链球菌,肺炎链球菌,新型隐球菌/格特隐球菌

2.病毒:巨细胞病毒,肠道病毒,疱疹单纯病毒1,疱疹单纯病毒2,HHV6,人双埃克病毒,水痘-带状病毒

FDA提醒:FilmArray ME Panel不能识别所有CNS感染,也不能准确确定抗菌剂对细菌的针对性。因此,临床医生应该进行标准CSF细菌和真菌培养,因为新型测试会产生假阳性或假阴性结果。另外,如果出现阳性结果,需要进行药物敏感性测试。

在一项怀疑为脑炎或脑膜炎的1560名患者的CSF样本的前瞻性研究中,与那些其他检测方法(包括脑脊液培养)得到的结果相比,FilmArray ME Panel检测得到的结果和他们有着高度的一致性。新的检测方法也证实了已知的存在于CSF样本中的病原体的准确性。
