当前,有研究正在探究DNA-次甲基化药物与其他表观遗传学药物,如组蛋白去乙酰化酶抑制剂或分化诱导剂(如视黄醇)联合治疗骨髓瘤的效果。我们进行了一项2×2析因设计的随机II期试验,研究组蛋白去乙酰化酶抑制剂丙戊酸和全反式视黄酸(ATRA)对未接受治疗的老年急性髓细胞白血病(AML)患者的治疗效果。 200名不符合诱导化疗条件的患者(中位年龄,76岁;范围,61~92岁)仅接受地西他滨(20mg/m2静脉注射,第1~5天)(n=47)或与丙戊酸(n=57)、ATRA(n=46)或丙戊酸+ATRA(n=50)联合治疗。主要终点是客观反应,定义为完全缓解和部分缓解,以未接受治疗的老年急性髓细胞白血病(AML)患者中评估。关键次要终点是总生存期、无事件生存期和无进展生存期和安全性。 结果显示,加入ATRA后,缓解率更高(ATRA为21.9%,无ATRA为13.5%;OR为1.80;95%CI,0.86-3.79;单侧P=.06)。对于丙戊酸盐,没有观察到效果(丙戊酸盐为17.8% v 无丙戊酸盐为17.2%;OR,1.06;95% CI,0.51-2.21;单侧P = .44)。使用ATRA的中
