Radiology:CT测量肾体积在常染色体显性多囊性肾病中的价值

背景,对常染色体显性多囊性肾病(ADPKD)肾体积(TKV)评价是非常有价值的,但MRI测量参考标准需进行MRI扫描且花费时间较长。本研究旨在验证利用可选择性低剂量CT和省时等价测量方法所得准确TKV能与参考标准MRI测量结果相媲美。

背景,对常染色体显性多囊性肾病(ADPKD)肾体积(TKV)评价是非常有价值的,但MRI测量参考标准需进行MRI扫描且花费时间较长。本研究旨在验证利用可选择性低剂量CT和省时等价测量方法所得准确TKV能与参考标准MRI测量结果相媲美。

本研究共纳入了ADPKE且行1次MRI和2次CT检查的患者。低剂量(LD)和超低剂量(ULD)CT检查的辐射剂量分别在1.4和2.6以下,要低于场馆腹部和盆腔CT扫描。利用基于模型迭代重建方法重建ULD CT图像。采用3中TKV测量方法对所有图像进行测量,以MRI人工测量结果为参考标准。利用Spearman相关性分析、简单线性回归及均方根误差(RMSE)计算分析三种方法的准确性。利用ICC检验可重复性。

结果为,30例受试者平均TKV为1368.9 mL ± 1146.13。ULD和LD CT方案中位数剂量长度成绩分布为58.8、115.5 mGy·cm(P = .01),CT剂量指数分别为1.2、3.9 mGy。所有成像方案和测量方法与参考标准均具有良好相关性(r2 > 0.98)。RMSE范围为 80.5-157.3 mL (5.9%-11.5% 平均TKV)。所有方法ICC均高于0.98。平均测量时间范围为4.6 to 5.2分钟,要明显短于人工测量结果(27.7分钟,范围为14-60分钟)。

本研究表明,利用低剂量CT方案和基于线性计算的体积测量与参考标准相比具有相似的准确性和可重复性。
