PLOS MED:锂盐使用能降低痴呆及其亚型发生率风险

痴呆症是老年人群的主要死因。

痴呆症是老年人群的主要死因。预防性的干预措施,即使能适度延迟痴呆症的发生,也会对公共健康产生重大影响。目前还没有改变疾病的治疗方法。锂已被提议作为一种潜在的治疗方法。

为了评估锂的使用与痴呆症及其亚型的发病率之间的关系,来自英国剑桥的学者开展了一项回顾性队列研究,比较了2005年1月1日至2019年12月31日期间接受治疗的患者数据,结果发表字啊PLOS MEDICINE杂志上。

使用的数据来自英国剑桥郡和彼得堡NHS基金会信托(CPFT)的二级护理精神卫生(MH)服务的电子临床记录。符合条件的患者为基线年龄在50岁以上,并有至少1年的随访。干预措施是使用锂。主要结果是痴呆症及其亚型,根据《国际疾病分类-第十版》(ICD-10)进行诊断和分类。

该队列纳入了29618名患者(其中548人使用了锂)。他们的平均年龄为73.9岁,40.2%为男性。锂暴露的患者更有可能是已婚、同居或存在伴侣,目前/曾经吸烟,使用过抗精神病药物,并合并有抑郁症、躁狂症/双相情感障碍(BPAD)、高血压、中心血管疾病、糖尿病或高脂血症有关。

在其他特征方面,包括年龄、性别和与酒精有关的疾病,两组之间没有明显差异。在暴露锂盐的队列中,53名(9.7%)患者被诊断为痴呆症,包括36名(6.8%)阿尔茨海默病(AD)和13名(2.6%)血管性痴呆(VD)。在控制混杂因素后,使用锂与较低的痴呆风险有关(HR=0.56,95%CI 0.40-0.78),包括AD降低45%(HR=0.55=),VD降低67%(HR =0.36)。

综上,锂的使用能降低痴呆的风险,这进一步支持了锂可能是一种治疗痴呆症的方法。
