脑容量越大,脑中风预后越好!

前循环的中度和重度卒中占缺血性卒中的很大一部分,如果不及时治疗,功能结局不佳。

在脑卒中患者中,长期结局难以预测。NIHSS评分测量的初始卒中严重程度、弥散加权成像(DWI)的缺血病灶体积、病灶位置等特征与卒中结局密切相关,但全局性的脑健康也可能影响康复。在这种情况下,有人认为,要使卒中后的恢复过程达到最佳功能,需要保持整个大脑的细胞、结构和血管的完整性。小血管改变的缺失和较大脑容量的存在是脑健康的影像学标志.,一般中风前的特征,包括白细胞增多症、脑萎缩和陈旧性血管病变/梗塞、腔梗(lacune)的存在,与不良结局独立相关,可能代表了脑衰弱的标志。

此外,根据基于人群的研究,较小的脑容量可能与未来较高的卒中风险相关。这些影像学特征常见于腔隙性卒中和脑小血管疾病(SVD)患者。以往的研究主要集中在单个SVD特征或SVD总负担对卒中后功能结果预测的影响,并使用视觉评分表来评估萎缩情况。

这些研究大多认为SVD标志物和萎缩可能会阻碍卒中后的功能恢复和神经认知轨迹,很少有研究使用适合脑容量测量的磁共振成像序列,如3D T1磁化预备-快速梯度回声( Magnetization Prepared-Rapid Gradient Echo),因为时间限制,这些通常不会在卒中的超急性期获得。这可能阻碍了对全局脑容量在超急性非急性卒中中的作用的研究。


藉此,奥地利弗洛里神经科学和心理健康研究所的Ning Bu等人,回顾性地研究了参加AXIS-2试验的急性缺血性卒中患者,AXIS-2试验是粒细胞集落刺激因子与安慰剂的随机对照临床试验。

他们使用SPM12中的统一分割算法,从症状发生9小时内获得的常规基线磁共振成像数据中评估脑实质体积与脑内总体积(即脑实质部分)和脑总体积的比值。

血管周围空间扩大、白质高信号病变、腔梗(lacunne)以及小血管疾病负担,均以目测量化评定。并测定90天时的功能结果(改良Rankin量表评分)。Logistic回归用于测试脑成像特征和功能结果之间的关联。

他们纳入259名患者,平均年龄69±12岁,46 %为女性。

脑实质分数的增加与更高的优良结局几率相关(OR 1.078)。脑总体积和小血管疾病负担与功能结果无关。未观察到脑实质分数和大血管闭塞对优良结局的交互作用。

这个研究的重要意义在于,发现了磁共振成像上的脑实质分数可用于评估全脑健康,其与缺血性卒中后的优良功能结果相关。
