Mov Disord-随访十年,亨廷顿患者的运动,认知和影像学如何变化?

亨廷顿病(HD)是一种神经退行性疾病,由亨廷顿(HTT)基因中的胞苷-腺嘌呤(CAG)片段的常染色体显性扩增引起,以运动、认知和精神功能的逐渐衰退为特征。目前还没有改变疾病的治疗方法,但关键性的试验正在进行中。十多年来的研究有助于改善临床试验设计,确定试验结果的组别差异,对与HD相关的变化具有高度敏感性。

HD最早是以漫长的无症状前期为标志,时间长达数十年,随后是疾病严重程度的全面表现。漫长的发病前期意味着在整个谱系中存在相当多样的生物物理和生理因素,这些因素可能会转化为广泛不同的体征/症状概况,这也可能在未来治疗的反应中发挥作用。

因此,有效进行新的临床试验的最关键障碍之一是如何将异质个体分配到同质亚组。通过选择具有特定症状的参与者,找到未来药物干预的最佳窗口,确定有效的HD分层将对治疗的发展具有重要意义。

整合HD相关的各个研究(PREDICT-HD、TRACK-HD/TRACK-ON和IMAGE-HD)将会有更大的样本样本,而且能纳入多个地理位置、不同遗传背景的参与者。这种方法可以对HD人群中更具代表性的样本进行研究,并进行跨研究的验证,以提高结果的稳健性。

此外,结合这些数据集可以捕捉到更广泛的疾病进展片段(例如,尽管IMAGE和TRACK在1组中有少量参与者,但PREDICT提供了相同的更大的参与者样本),因此可以改善特征描述的范围。结合不同的数据集已被证明是富有成效的,如研究脑部疾病的ENIGMA联盟就说明了这一点。ENIGMA获得了在全球不同地点收集的小样本数据,涵盖了广泛的脑部疾病,汇总后生成大数据集,以提高研究结果的质量、普遍性和稳健性。

藉此,澳大利亚Monash大学的Pubu M. Abeyasinghe等人,结合了来自IMAGE、TRACK+TRACK-ON(以下简称TRACK)和PREDICT的数据集,通过结合运动、功能、认知和形态成像标志物,系统探究HD的进展。

他们将IMAGE-HD、TRACK-HD和PREDICT-HD研究中的1082名参与者的临床和形态成像数据结合起来,纵向时间在1-10年之间。使用CAG和年龄产品将参与者分为4组。使用多变量线性混合模型,测试了63种标记物组合在区分CAG和年龄乘积组方面的敏感性。

他们发现:壳核和尾状核团的体积,能区分CAG的年龄分组,而且是能反应进展的最佳变量。

如果只使用尾状核的体积,统计模型最能描述综合数据中的晚期疾病进展情况。

与预期相反,结合临床标志物和体积测量并不能改善追踪纵向进展。

这个研究的重要意义在于发现了 : 在整个研究过程中,监测体积变化,同时与主要和次要的临床终点一起,有助于增加人们对功能结局改善的认识,并有助于改善临床试验的设计。
