砒霜或可用于系统性红斑狼疮的治疗

砒霜（ATO）是最古老的毒物之一，同时也是一味中药。法国南特大学的研究团队的研究显示，小剂量静脉注射ATO治疗系统性红斑狼疮（SLE）有较好的疗效。

砒霜（ATO），别称信石、红矾、白砒等，是最古老的毒物之一，由于当时技术上的限制，砒霜中常混有大量的硫或硫化物而呈红色，故又称为“鹤顶红”，其主要成分为三氧化二砷（As₂O₃）。同时，砒霜也是一味治病的药物，中医药学认为其味辛、酸，性热，归肺、脾、胃、大肠经，有蚀疮去腐、杀虫、劫痰、截疟等功效，可用于痔疮、瘰疬、痈疽恶疮、走马牙疳、癣疮、寒痰哮喘、疟疾、休息痢等病证的治疗。上世纪80年代，我国科学家首次证明其对急性早幼粒细胞白血病（APL）有治疗作用。近日，来自法国南特大学的研究团队在Arthritis Research & Therapy上发表的一项研究显示，小剂量静脉注射ATO治疗系统性红斑狼疮（SLE）有较好的疗效和安全性。

这是一项开放性多中心II期临床试验，纳入2013年7月至2015年10月期间，来自法国6家大学医院的11名活动期SLE患者，所有患者免疫荧光抗核抗体（ANA）均≥1:80，在接受常规的激素、免疫抑制剂治疗后，SLE疾病活动指数（SLEDAI）评分仍＞6分。患者的ATO给药方案是根据先前从APL治疗中获得的药代动力学数据设计的，前4天每天给予1次负荷剂量注射，然后第2到4周每周给予2次注射，每次剂量0.1mg/kg~0.2mg/kg，整个疗程24天，共进行10次静脉注射。

对所有11名患者进行安全性评估，未发生剂量限制毒性（DLT），有2名患者因治疗相关的严重的不良事件（SAE）在7次注射后停止了ATO治疗。有18人次不良事件（AEs）被认为与研究药物有关，但均为轻到中度。

第16周时，SLEDAI中位评分从基线时8分下降到3.4分。第24周时，口服激素中位剂量从基线时的12.25mg/d下降到6mg/d，7例患者达到了7.5mg/d的目标剂量，有5例患者获得SLE反应指数4（SRI-4）应答。

6例基线期抗双链DNA（dsDNA）抗体阳性的患者，观察到从第4周到第8周出现dsDNA水平有下降趋势。补体C3、C4和IgG、IgA、IgM、抗Ro/SSA抗体、抗心磷脂抗体水平没有变化。

在本临床试验中，短疗程静脉注射ATO对SLE患者显示出了令人鼓舞的疗效和可以接受的安全性。这些初步结果支持在更大的临床试验中进一步更好的评估ATO对SLE及其他系统免疫性疾病的获益-风险。
