《科学》封面重磅:一个关于T细胞悲伤而重要的研究

好像有这么一个怪现象:在男女比例严重失调,且美女如云的专业或者院系,大部分男生都没有女朋友。据说这是「审美疲劳」造成的。当然,你可能没发现这个现象,即使有这个现象,你可能也不认为是这个原因。但是,这些都不重要,重要的是,我希望你记住什么是「疲劳」。对于美女审美疲劳而言,大概就是美女见多了,看到女的基本就没啥兴趣了。好了,请记住「疲劳」这个概念。今天我要说的是,人体的T免疫细胞「疲劳」。你可能要

当然,你可能没发现这个现象,即使有这个现象,你可能也不认为是这个原因。但是,这些都不重要,重要的是,我希望你记住什么是「疲劳」。对于美女审美疲劳而言,大概就是美女见多了,看到女的基本就没啥兴趣了。好了,请记住「疲劳」这个概念。


今天我要说的是,人体的T免疫细胞「疲劳」。你可能要问了,这T细胞怎么会「疲劳」呢?那我反问一句,你说肝炎患者、艾滋病患者和肿瘤患者体内的免疫细胞,咋都不好好干活呢?明白了吗?它们也是累了。而且天天跟这些对身体有害的东西在一起,T细胞也习惯了(我隐约记得前不久有个研究发现,人体的免疫细胞只要已进入实体瘤,很快就会「懵逼」(1),这就好比我们进火锅店,几分钟之后,你基本就闻不到火锅味了)。这也是为什么上面的几种疾病都是慢性病的主要原因之一。

12月2日,顶级期刊《科学》以封面报道的形式,隆重介绍了Dana-Farber癌症研究所和宾夕法尼亚大学医学院团队,关于T细胞疲劳(T cell exhaustion,也译作:T细胞耗竭)的两篇独立重要研究。这两篇研究意义重大,因为它们给免疫检查点抑制剂(PD-1和PD-L1抗体)治疗响应率低,CAR-T治不了实体瘤提供了一个重要的解决方案。

T细胞疲劳现象于多年前在小鼠体内率先发现(2; 3),后来科学家在小鼠和人的多种慢性疾病模型中都发现了这个现象(4)。这些疲劳的T细胞的表现是,基因表达与正常的不一样了,杀敌的武器也没了,抑制它起作用的因子却越来越多了。很有一种征战沙场多年之后,解甲归田,不问世事的意味。


那好好的T细胞为何会这样呢?我们有没有办法给它打一针鸡血,让他重新征战沙场呢?


还真有,免疫检查点抑制剂中的PD-1和PD-L1抗体就是解除T细胞对自己的抑制的;CAR-T就是给T细胞重新装上武器的。这两种疗法这几年的确带来了激动人心的研究成果,但是如同前文所言:目前,免疫检查点抑制剂(PD-1和PD-L1抗体)治疗响应率低,CAR-T还治不了实体瘤。受益者终归是一小撮患者。


要解决问题,还得靠深入的基础研究。


我们先来看看Dana-Farber 癌症研究所W. Nicholas Haining博士和加州大学伯克利分校Nir Yosef博士团队的发现(5)。

Haining团队的Debattama R. Sen和Yosef团队的James Kaminski是这项研究的第一完成人。他们就想啊,既然疲劳的T细胞和活力四射的T细胞之间差异这么大,那我们就看看它们调节基因表达的区域有啥区别。


于是,他们就用了号称可以鉴定基因组中所有活跃的调控序列的ATAC-seq技术(6)。真是不看不知道,一看吓一跳。原来每时每刻调节疲劳的T细胞和活力四射的T细胞基因表达的原件,有44.48%处于不同的状态。难怪差异如此之大。


「我们通过这项技术发现了疲劳的T细胞和活力四射的T细胞之间最根本的区别,」Haining说,「那些在疲劳的T细胞中出现的表达区域,在活力四射的T细胞没有出现;反之亦然,这样的例子可以举几千个。这就告诉我们,这两种细胞实际上是两套不同的信号通路。」



Juno的CAR


现在的CAR-T治疗都是简单的给T细胞加上识别肿瘤细胞的武器,而且在血液肿瘤中发现CAR-T的确可以很好的识别癌细胞。但是CAR-T还是会很快就处于疲劳状态。简单了说就是,目前的CAR-T没有从根本上解决问题。这估计也是免疫细胞一进入实体肿瘤内,就进入疲劳状态的主要原因。


实际上,细胞内部很多基因的表达是互相调控的,打开或者关闭某个基因,可能会影响很多基因的开关。「堵上恼人的PD-1,或者破坏PD-1基因,肯定不如找到某个关键的调节基因,通过改变这个位点,扭转整个细胞的状态。」Haining在接受STAT采访时说,「更何况PD-1对人体还有好的一面呢。」


这就是Haining博士和Yosef博士团队的研究成果,给CAR-T提出的研究方向。那免疫检查点抑制剂PD-1和PD-L1等抗体该咋办呢?


再让我们来看宾夕法尼亚大学医学院E. John Wherry博士团队的研究(7)。



T细胞发展的两个方向,上面一支是在急性感染中,蓝色的T细胞很有杀伤力;下面一支表示在慢性疾病中,T细胞被反复折磨最终疲劳而死。


与Haining博士团队的研究相比,Wherry博士团队的研究更加具体,直接具体到PD-1/PD-L1这个信号通路上了。


Wherry博士发现,虽然阻断PD-1可以在一定程度上重新激活疲劳的T细胞,并改善对慢性感染和癌症的症状;但是,这种被激活的T细胞在遭受新一轮抗原的刺激之后,马上又进入疲劳状态。这就解释了为什么很多人在接受免疫检查点抑制剂药物的治疗之后,没过多久病情就会反弹的原因。


为了了解清楚反弹背后的原因。Wherry博士也用ATAC-seq技术分析了疲劳T细胞在接受免疫检查点抑制剂阻断之后,基因表达调控元件的表观遗传学变化。结果发现,使用PD-L1抗体阻断PD-1/PD-L1信号通路,根本就没有改变Haining博士发现的那种基因表达状态。


「我们认为,我们的研究表明,PD-1/PD-L1等免疫检查点抑制剂的疗效,可能受制于疲劳T细胞表观遗传学层面不可逆的变化。」Wherry实验室的博士后研究员,论文的第一作者Kristen E. Pauken博士说。



PD-L1抗体与PD-L1结合


虽然PD-L1抗体能暂时改变T细胞疲劳的状态,但是疲劳细胞在表观遗传学层面的改变,PD-L1抗体是没法逆转的。这真是一个悲伤的故事。


澳大利亚莫纳什大学的Stephen J. Turner和Brendan E. Russ在本期《科学》杂志发表评论性文章(8)。他们认为,Dana-Farber癌症研究所和宾夕法尼亚大学医学院团队的发现,虽然解释了为什么免疫检查点抑制剂治疗为啥会失败,但是PD-1/PD-L1抗体也有很多成功的例子呀,虽然是少数。Turner和Russ也给出了自己的猜想,是不是因为这些人的T细胞都处在可逆转可暂停的疲劳早期阶段呢?同时,他们还提出了另一个问题,这种现象对其他的免疫检查点抑制剂是不是也一样,对其他的肿瘤类型呢?


搞清楚这些问题对于推进免疫治疗的发展大有裨益。如果他们的研究成果是广泛存在的,那么我们现阶段就可以做一些改变。例如我们是不是可以利用这个发现设计一套检测方案,用于区分免疫检查点抑制剂效果不好和效果好的肿瘤患者;在给癌症患者做CAR-T治疗时,研究人员可以用这种方法筛选不是那么疲劳的T细胞做改造;再进一步,可以开发一些针对表观遗传学的药物,进一步找到T细胞发生疲劳的深层次原因等等。研究人员应该可以开发出更持久有效的癌症治疗方法。
