近期,一项发表在杂志Kidney Int上的研究对这一问题进行了探讨。研究者们评估了一组儿科肾移植受体患者中的AT1R抗体与临床结果、活检结果、炎性细胞因子和HLA供体特异性抗体(DSA)的关系。纵向监测了65例患者的AT1R抗体、HLA DSA、IL-8、TNF-α、IL-1β、IFN-γ、IL-17和IL-6水平,肾功能异常、高血压、排斥反应以及移植后前两年的同种异体移植物丢失。 

此项研究结果显示:在65例儿童中的38例患者出现AT1R抗体阳性,但其与HLA DSA无关。 AT1R抗体与肾同种异体移植物丢失(优势比为13.1 [95%置信区间1.48-1728]),存在肾小球炎或动脉炎,TNF-α、IL-1β和IL-8水平显着升高相关,但与排斥或高血压无关。在有或无排斥反应的患者中,AT1R抗体与eGFR显着下降相关。此外,在为期两年的随访期间,在没有排斥反应的患者中,AT1R抗体是eGFR恶化的重要危险因素。

由此项研究发现,AT1R抗体与同种异体移植物中的血管炎症,eGFR逐渐下降,同种异体移植物的丢失相关。AT1R抗体和炎性细胞因子可以鉴定那些具有肾血管炎症风险的患者,由此可以尽早进行活检和干预。
