Hypersensitivity Pneumonitis

 
概述
  • 吸入各种抗原颗粒引起的弥漫性肉芽肿性间质性肺疾病

X线：

  • 急性/亚急性: 正常或中、下肺结节

  • 慢性: 中、上肺纤维化 (结构扭曲，容积减少) 

HRCT：

  • 磨玻璃影 (100%) 
  • 小叶中心磨玻璃结节 (70%) 
  • 呼气空气潴留 (95%) 
  • 肉皮冻征：磨玻璃影+正常肺+空气潴留
  • 慢性：中、上肺纤维化
 

(左) 示意图：过敏性肺炎的典型特征，表现为慢性多灶性、双侧、弥漫性小叶中心、界限不清的磨玻璃结节。鉴别诊断包括呼吸性细支气管炎。
(右) 呼吸困难患者，轴位HRCT显示小叶中心磨玻璃结节，提示为过敏性肺炎。结合详细的病史和体格检查对确定诊断至关重要。

(左) 急性过敏性肺炎患者的轴位HRCT显双肺弥漫性磨玻璃影和左肺上叶肺大泡。
(右) 过敏性肺炎患者的呼气相HRCT显示双肺部分小叶区域空气潴留，空气潴留合并斑片状磨玻璃影 (小叶中心分布) 高度提示过敏性肺炎。

 

 

(左) 过敏性肺炎患者的冠状CT显示双肺弥漫性斑片状磨玻璃影和散在的低密度区。

(右) 一例过敏性肺炎患者的冠状呼气HRCT显示小叶空气潴留（箭）和相对于正常背景肺的高密度区域（空箭）。这3种不同肺密度的共存 (通常有地理边缘) 被称为肉皮冻征（head cheese sign），高度提示过敏性肺炎。

(左) 轴位HRCT显示小叶中心和融合的磨玻璃影（箭），另可见小叶样的密度减低区（空箭）。
(右) 同一患者，呼气HRCT (在同一位置获得) 显示小叶空气潴留（空箭）、密度增高（箭）和正常密度区域，呈肉皮冻征。结合临床病史对于确定过敏性肺炎患者抗原暴露的病因至关重要。

(左) 一例过敏性肺炎患者的轴位HRCT显示轻度小叶中心磨玻璃影。在鉴别诊断中还应考虑吸烟相关的呼吸性细支气管炎。
(右) 慢性过敏性肺炎患者的冠状HRCT显示双肺上叶纤维化，表现为网状影、结构扭曲、牵拉性支气管扩张和蜂窝征。斑片状磨玻璃影区域可能代表亚急性过敏性肺炎的剩余异常表现
