最近,有研究人员调查了尿精胺和精胺风险评分在预测高等级前列腺癌(HGPCa,ISUP等级组≥2)中的作用

研究人员从两家医院前瞻性地招募了905名PSA升高的连续男性。该队列中分别有30.8%(185/600)和17.2%(103/600)的男性诊断为PCa和HGPCa,并与较低的尿精胺水平显著相关。在精胺结果的最低和最高四分位数之间,PCa风险增加了3倍(49.3% vs. 16.7%),ISUP等级组≥2的PCa风险增加了3.5倍(31.3% vs. 8.7%)。多变量分析表明,PSA、前列腺体积(PV)、数字直肠检查(DRE)和精胺是PCa和HGPCa的独立预测因子,精胺风险评分与上述因素能够获得最高的AUC,PCa和HGPCa分别达到为0.78和0.82。对HGPCa的敏感性为90%,可以避免36.7%的活检和24.4%的ISUP 1级组诊断,阴性预测值为95.4%。DCA显示精胺风险评分具有临床净获益。内部验证与bootstrapping显示了良好的识别性和校准性。

最后,研究人员指出,尿液精胺和精胺风险评分能够确定HGPCa风险较高的男性,并减少不必要的活检
