盘点:近期“维生素D”相关研究汇总

【1】原发性甲状旁腺功能亢进症患者的维生素D研究

研究纳入了50名原发性甲状旁腺功能亢进症成年患者(平均年龄58.3岁)和50名年龄、性别和体重指数相匹配的对照组(平均年龄58岁)。

36%的原发性甲状旁腺功能亢进症患者,其25(OH)D水平低于20 ng/mL。与对照组相比,原发性甲状旁腺功能亢进症患者的总25(OH)D水平和维生素D结合蛋白水平更低(P < .01)。

两组参与者有相似的白蛋白结合25(OH)D水平;但是对照组维生素D结合蛋白25-(OH)D水平更高(P < .01)。原发性甲状旁腺功能亢进症患者的白蛋白水平低于对照组(P < .05)。

研究者发现,血清维生素D结合蛋白水平和甲状旁腺激素、钙水平之间呈负相关(P < .01);血清维生素D结合蛋白水平与25-(OH)D(P < .05)和白蛋白水平(P < .05)水平之间呈正相关。

研究结果表明,总25(OH)D水平可能不能够准确的反应原发性甲状旁腺功能亢进症患者的生物有效性25羟维生素D水平。临床医生应该意识到,原发性甲状旁腺功能亢进症患者出现低水平25-(OH)D,可能存在高钙血症恶化。(文章详见——Endocr Pract:原发性甲状旁腺功能亢进症患者的维生素D研究

【2】维生素D和NAFLD的严重程度无关

研究者对283个研究对象进行了分析研究,其中227个白人、45个黑人、9个西班牙裔人、2个亚洲人。这项研究包括244例NAFLD患者,余下的为对照组。

数据显示,NAFLD患者更有可能存在更高的糖化血红蛋白水平(6.5 ± 1.2 vs. 5.9 ± 1; P = .009);来自代谢和减肥手术计划的患者更不可能存在脂肪肝活检证实NAFLD (P = .008);此外,Patel和同事们发现,NAFLD组患者和对照组参与者的25羟基维生素D水平没有差异(27.6 ± 11.8 ng/mL vs. 27.9 ± 12.8 ng/mL; P = .88)。

以下参与者的维生素D水平更高:使用维生素D补充剂(n = 93; 31.2 ± 13.0 vs. 25.9 ± 10.9; P = .0003)、肤色浅(28.4 ± 11.6 vs. 24.6 ± 12.8; P = .039)。研究人员还指出,与深肤色参与者相比,浅肤色的NAFLD比例更大(与对照组相比)(83.6% vs. 64.1%; P = .005)。(文章详见——Am J Gastroenterol:维生素D和NAFLD的严重程度无关

【3】维生素D不足,更容易出现认知障碍?


该研究纳入了2273名年龄≥60岁的参与者,来自SEED研究,平均年龄70.4±6.2岁,女性占44.7%,根据25OHD浓度进行分类:≤10、10.1–20、>20 ng/mL。使用简易智力测试(AMT)评估参与者的认识能力。0-6年教育水平人群的AMT ≤6或>7年教育水平人群AMT≤8,则定义为全球认知功能障碍。荟萃分析中包含了7篇相关研究,均为亚洲研究,涉及6068名参与者,其中1179例认知功能受损。

25OHD浓度>20 ng/mL(n = 1302)的人与更低25OHD浓度的人相比,有更高的AMT总分数(平均 8.5 ± 1.9),更不可能出现认知障碍(14.1%)(所有P< 0.001, P-trend < 0.001)。消除季节变动影响后的25OHD浓度与AMT评分相关(β = 0.10/10 ng/mL, P = 0.035)。维生素D不足(25OHD ≤20 ng/mL)与全球认知障碍相关(OR 1.56, P = 0.028)。具体来说,25OHD浓度与语义记忆(r = 0.08, P = 0.009)和定时(r = 0.09, P = 0.003)有关。

荟萃分析中,认知障碍患者与认知健康的对照组相比,平均25OHD浓度相差−6.83 ng/mL(95% CI −11.36; −2.30),表明认知障碍患者有更低的25OHD水平。

【4】应在夏天检测25羟维生素D,而不是在冬天

研究纳入了5002名瑞典妇女(平均年龄68岁),来自于一项以人群为基础的大型纵向队列研究,所有参与者都有重复测量的饮食和生活方式相关信息。参与者在2003–2009年进行临床检查,包括使用双能X线骨密度仪测量骨密度、采集空腹血液标本测量S-25OHD(使用HPLC-MS/MS进行测定)等。

结果显示,夏天收集的样本(占队列的6%),全髋关节骨密度逐步增加,S-25OHD水平高达40 nmol/L。在夏天,与S-25OHD>80 nmol/L的女性相比,S-25OHD<30 nmol/L的女性,其校正后的BMD低11%(95% CI 3–19), S-25OHD在30到40 nmol/L的女性,其校正后的BMD低6%(95% CI 1–11)。在夏天,与S-25OHD>80 nmol/L的女性相比,S-25OHD<30 nmol/L的女性,其校正后的骨质疏松相对风险更高(RR 4.9; 95% CI 2.9–8.4)。对于冬天,没有发现类似夏天的这些联系。

【5】环境中的邻苯二甲酸盐和双酚A也会影响维生素D?

研究者使用美国全国健康和营养调查(2005 - 2010)数据,进行了横断面研究,探究尿液中11种邻苯二甲酸代谢物浓度、BPA和血清总25(OH)D水平间的关系。

对于总人群和性别分层模型而言,邻苯二甲酸二异辛酯(DEHP)的代谢产物与总25(OH)D水平呈负相关。在总体人群中,我们检测到DEHP代谢物的摩尔总和(ΣDEHP)与总25(OH)D水平呈负相关(ΣDEHP增加1个四分位数范围,总25(OH)D水平下降1.90%;95% CI -3.64, -0.17)。邻苯二甲酸单乙酯则与总25(OH)D水平呈正相关。对于女性,BPA与总25(OH)D水平呈负相关,尿液中BPA增加1个四分位数范围,总25(OH)D水平下降3.71%(95% CI, -6.41, -1.02);但是对于男性,不存在这种关系。

【6】增加血浆维生素D水平,或许可以降低卵巢癌风险

与循环中25羟维生素D[25(OH)D]浓度有关的单核苷酸多态性有如下几个:rs7944926 (DHCR7)、rs12794714 (CYP2R1)和rs2282679 (GC)。本研究纳入了31 719名欧洲血统的妇女(10 065例卵巢癌患者,21 654例对照组),均来自卵巢癌协会联盟;使用定制的Illumina Infinium iSelect (iCOGS)阵列对参与者的基因进行分型。使用双样本(汇总数据)MR方法对所有卵巢癌患者(10 065例)和高级别浆液性卵巢癌患者(4121例)分别进行分析。

数据显示,对于上皮性卵巢癌(10 065例),使用逆方差加权,结合个体单核苷酸多态性,25(OH)D浓度每下降20 nmol/L,上皮性卵巢癌风险的比值比(OR)为1.27(95% CI 1.06~1.51);对于高级别浆液性上皮性卵巢癌(4121例),该OR值为1.54 (1.19, 2.01)。

【7】维生素D和体重有什么关系?

研究者对随机和非随机对照试验进行了一项系统回顾和荟萃分析,比较体重下降 vs 体重维持,是否会导致血清25羟维生素D的增加。

对相关数据库进行检索,截止时间2016年3月31日。纳入了以肥胖和血清25羟维生素D变化作为主要或次要终点的研究。

我们确定了4项随机对照试验(n = 2554)和11项非随机对照试验(n = 917),纳入荟萃分析。体重下降 vs 体重维持组,血清25羟维生素D浓度增加(组间相差 3.11 nmol/L (95% CI: 1.38, 4.84 nmol/L));非随机对照试验中,我们观察到组间差距为4.85 nmol/L (95% CI: 2.59, 7.12 nmol/L)。没有发现体重下降与血清25羟维生素D增加的剂量反应关系。

我们的研究结果表明,在相同维生素D补充剂的条件下,体重下降可略微改善维生素D的状态(相比体重维持组)。虽然在未来还需要更多的研究来证实这些发现,但目前我们的研究结果暗示了,肥胖与低血清25羟维生素D之间的关系,可能是因为肥胖导致循环中维生素D适度下降所致。(文章详见——Am J Clin Nutr:维生素D和体重有什么关系?

【8】补充维生素D会不会增加肾结石风险?

研究者对维生素D补充剂(干预组) vs 安慰剂组(对照组)(≥24周)的RCT试验数据进行了分析,调查了钙代谢相关副作用,尤其是高钙血症、高钙尿症、肾结石。检索相关数据库后,一共纳入了48项研究、涉及19833例参与者,对其进行数据合并荟萃分析。

只有9项试验有肾结石相关报告,且干预组肾结石发生率低于对照组,不过不具有统计学意义(RR: 0.66, 95% CI: 0.41, 1.09; P = 0.10)。37项研究数据表明,干预组 vs 对照组,参与者高钙血症风险增加(RR: 1.54; 95% CI: 1.09, 2.18; P = 0.01)。14项研究数据表明,干预组 vs 对照组,参与者高钙尿症风险增加(RR: 1.64; 95% CI: 1.06, 2.53; P = 0.03)。

亚组分析表明,维生素D补充剂对高钙血症、高钙尿症、肾结石风险的影响,与基线25羟维生素D、维生素D剂量和持续时间或同时补充钙等因素无关。

结果表明,长期补充维生素D可能会导致高钙血症和高钙尿症的风险增加;但并不会增加肾结石的风险。还需要大型的随机对照试验来证实这些发现。(文章详见——Am J Clin Nutr:补充维生素D会不会增加肾结石风险?

【9】补充维生素D可减少严重哮喘的发作

研究人员着手进行了一项关于持续12周的安慰剂对照试验的荟萃分析以评估维生素D对严重哮喘加重的影响。回顾文献后研究人员纳入七个试验,其中包括435名儿童,以及两项研究,涉及658名成人。大多数研究对象有轻度至中度哮喘,研究持续了4-12个月。

结果表明服用维生素D减少了至少一次需要去医院急救部门和/或入院治疗的哮喘加重的风险(比值比为0.39(95%可信区间为0.19-0.78))。需要补充维生素D治疗以避免严重的哮喘发作的人数为27。

维生素D的补充也降低了需要全身性类固醇治疗的哮喘加重的比例(率比0.63(0.45-0.88))。但是对肺功能或日常哮喘症状的影响很小或没有。
