TCT 2020丨PROSPECT Ⅱ/ABSORB研究:易损斑块的预防性PCI?

在TCT Connect 2020上公布的PROSPECT Ⅱ和PROSPECT ABSORB研究结果显示,经血管内超声(IVUS)和近红外光谱(NIRS)鉴定的易损斑块患者发生重大心血管不良事件的风险显着增加,但对这些富含脂质的病变进行预防性治疗似乎能提供一些保护,至少在扩大管腔面积和可能预防临床事件的方面。

PROSPECT-ABSORB研究是前瞻随机对照试验PROSPECT Ⅱ的一部分,研究对存在易损斑块的患者行冠状动脉介入治疗(PCI),置入现已停用的生物可吸收支架(BVS),经过2年多的随访显示最小管腔面积(MLA)明显大于未置入支架的易损斑块。

主要研究者、纽约西奈山伊坎医学院Gregg Stone博士表示,该研究是首次随机比较预防性血运重建术和指南指导下的药物治疗(GDMT)对于轻度/无血流限制,且有较大斑块负荷病变的影响。成像技术的进步,包括IVUS、NIRS,甚至光学相干断层扫描(OCT),能够识别无血流受限的易损斑块。然而,这些具有薄纤维帽的富含脂质的斑块是否值得治疗还不得而知。

问题是如何处理他们?Stone 指出,我们想知道是否可以安全地治疗这些斑块并扩大管腔。之所以选择Absorb BVS进行血运重建,是因为已发表的研究表明,这种生物可吸收支架能够诱导内膜增生,并在本质上密封薄纤维帽的动脉粥样硬化斑块,将富含脂质的坏死核心与管腔分离。新生内膜增生会使纤维帽变厚,使壁应力正常化。这会使斑块稳定下来。当然,动脉栓塞术中富含脂质的病变也有风险,并可能引起再狭窄、支架血栓形成和其他并发症,如果不治疗它们就不会发生。

PROSPECT ABSORB研究

在PROSPECT Ⅱ中,纳入898例近期ST段抬高型心肌梗死或肌钙蛋白阳性NSTEMI患者,所有患者均接受了三支血管的IVUS+NIRS血管成像检查。

其中,PROSPECT ABSORB研究者随机选择了182名非罪犯病变处存在至少一个无血流受限且斑块负荷≥65%病变的患者接受BVS或药物治疗。Stone介绍道,之所以选择这个IVUS阈值,是因为它与之前实验室评估的斑块负荷相关,既往研究表明,该阈值可以独立预测PROSPECT Ⅰ研究中随后的病变相关事件。尽管PCSK9抑制剂的使用有限,药物治疗组的所有患者均接受他汀类药物治疗,大多数患者接受高强度他汀类药物治疗。

主要安全终点是心源性死亡、靶血管心肌梗死或临床驱动的TLR,BVS治疗组和药物治疗组的主要终点发生率分别为4.3%和4.5%(P=0.96)。

25个月时,BVS治疗组原位MLA从3.2增加到6.9 mm2,但接受药物治疗的患者没有变化。BVS治疗组和药物治疗组的MLA差异有统计学意义(P<0.0001)。BVS治疗组的患者在25个月时,整个病变(包括病变远端和近端5 mm边缘)的MLA也显着增大(5.2 vs. 2.9 mm2;P<0.0001)。BVS治疗的病变官腔在随访中有明显扩大,而用药物治疗的病变基本上是稳定的。

就临床疗效而言,BVS治疗组和药物治疗组在中位随访4.1年后病变相关的MACE事件发生率分别为10.7%和4.3%(P=0.12)。这种减少是由于严重心绞痛发作次数减少所致。BVS治疗组有1例血栓形成,这是由闭塞的侧支引起的,没有支架的参与。

该试验是一项初步研究,旨在评估未来的可行性,但Stone称较低的MACE发生率是一个“令人激动的”发现,值得在充分的随机试验中进行进一步研究。在这项研究中,BVS治疗的结果非常好,但在未来的试验中,我不会使用当前的BVS。支架内的晚期损失仍然是0.37 mm,这比我们预期的金属药物洗脱支架要高。虽然这种装置缺乏持久性对于易损斑块应用非常有吸引力,但我还是想要一个更先进的支架。我很乐意用最先进的金属药物洗脱支架进行关键的随机试验。”

治疗策略的成本-效果

美国盖瑟斯堡CVPath研究所Renu Virmani博士说,考虑到先前的病理学工作,NIRS- IVUS识别出的易损斑块相关的高风险并不令人惊讶。易损斑块基本上是前驱病变,即使不是非常狭窄也很危险。然而,目前她还没有完全相信,预防性治疗这种病变将减少未来的临床事件。事实上,在TCT 2020的其他专场还讨论了COMBINE OCT-FFR研究,Virmani评论说她更喜欢一种可以稳定高危斑块的无创疗法。

在PROSPECT ABSORB研究中病变相关的MACE获益是由较少的新发心绞痛引起的,这很难归因于干预治疗。“如果看看MACE的主要原因,就会发现它与进行性心绞痛有关。”Virmani说,“这是两组患者之间唯一的真正区别。这是可疑的。对患者完全设盲了么?他们知道自己做了些什么吗?”

加州大学欧文分校医学中心/长滩VA医学中心的Arnold Seto博士对这些新数据也持谨慎态度,他指出,初步研究只包括直径2.5-4.0 mm、长度不超过50 mm的血管。他补充说,这项试验对临床结果的支持不足,而且使用了一种已知与更多支架血栓形成相关的技术。在很大程度上支架植入术后25个月的MLA仍然较大并不奇怪。与Virmani一样,他指出病变相关的MACE终点可能会受到偏差的影响,因为患者被告知他们有一个无血流限制但高风险的病变,可能更容易出现心绞痛。这些评论与FAME 2 研究中提出的评论类似。

Stone表示,对于终点事件,新发的严重心绞痛是通过血管造影来判断的,并在随访中记录病变进展。他说:“如果对足够多的患者进行随机分组,确实会减少一些更严重的临床事件,但我们在这项试验中没有能力做到这一点。”

对Seto来说,PROSPECT ABSORB 是一个“勇敢(gutsy)”的试验,但它为心脏病学界打开了潘多拉的盒子。这项研究真的值得更大规模的临床试验吗?他问道,“可能与BVS无关,我关心的是显示差异所需的数字。”正在进行的1600名患者的PREVENT 试验可能会提供更多的证据来支持易损斑块假说。不过,从更大的角度来看,富脂斑块的预防性支架植入具有重要的政策意义,因为用NIRS-IVUS寻找易损病变既费时又昂贵,预防性支架植入术及其伴随的双联抗血小板治疗也是如此。除非研究人员能够识别出高风险病变,否则这种策略的临床益处可能很小。他还回顾了ISCHEMIA试验和其他稳定型心绞痛试验的经验教训。

“当血流动力学有显着变化的稳定型心绞痛患者支架的MACE率与最佳药物治疗的MACE率相似时,对于血流动力学无显着变化的心绞痛患者,是否能够期望预防性支架治疗在MACE发生率上的差异呢?很难想象这样的策略会比药物治疗更划算。我们应该把注意力集中在有症状和缺血的患者身上,就像FAME 2试验一样,”Seto说。

Seto补充道,三支血管内成像并非无风险,并发症发生率为1.6%。他还指出,当高危病变成为问题时,患者通常表现为心绞痛,而不是死亡或心肌梗死。换言之,在这些病变变得与临床相关之前,不去管这些病变可能是安全的。

另一方面,MedStar心脏和血管研究所Ron Waksman博士对最新研究结果更感兴趣,他说,PROSPECT-ABSORB研究证实了NIRS-IVUS检测到的富含脂质的斑块可以用BVS安全治疗的假设,而且很可能是DES中最好的一种,作为减少未来事件的潜在手段。

Waksman进行了富含脂质斑块(LRP)的研究,该研究表明NIRS可以识别无血流受限病变中的高危易损斑块,这项新的研究证实了LRP的研究,并预测这些新数据“可能为介入性心脏病专家开辟一个新的领域,将其作为一种二级预防措施来治疗易损的非狭窄病变。”然而,他强调,只有充分有效的临床试验才能决定是否应该提倡这种策略,像其他一样,PROSPECT ABSORB 只是一项安全性研究。

PROSPECT Ⅱ研究

在PROSPECT Ⅱ中,研究人员对805名患者的自然病史进行了评估,这些患者没有被随机分配到PROSPECT ABSORB研究,也没有接受最佳药物治疗。同样,这些患者都有一个或多个无血流受限病变,斑块负荷为65%,平均随访3.7年。

主要终点为心源性死亡、心肌梗死或不稳定型心绞痛/进行性心绞痛需要血运重建和/或证实病变进展的复合终点。4年中的发生率为13.2%。大多数事件发生在未经治疗的无血流受限病变(8.0%)中,而非罪犯斑块相关的MACE发生率为4.2%,低于以往研究。

当研究者根据NIRS评估的斑块负荷对非罪犯斑块相关的MACE发生率进行分层时,最大4mm脂质核心负荷指数(maxLCBI4mm≥325)上四分之一的患者有10.1%的MACE风险,而斑块负荷较少的患者有4.8%的MACE风险(OR 2.08;95%CI 1.18-3.69)。

“如果有一个高度富集的斑块,那么你患MACE的风险就会加倍,”首席研究员、瑞典隆德大学的David Erlinge博士说。当高危斑块被定义为斑块负荷≥70%的病变时,11.0%的患者有非罪犯斑块相关的MACE,而在斑块负荷较轻的患者中有3.6%的MACE。最后,当研究者将高风险斑块的两个特征——广泛的斑块负荷和一个大的富脂核心——结合起来时,这些个体与所有其他患者相比,有显着增加的非罪犯斑块相关的MACE的风险。例如,7.0%的患者同时具有这两种特征,而只有0.2%的患者没有这两种高危特征。“这种成像技术既可以发现高危斑块,也能发现那些可以推迟治疗的斑块,”Erlinge说。

德国慕尼黑的Adnan Kastrati医学博士对研究结果发表了评论,称PROSPECT Ⅱ研究是一项重要的研究,但他补充说,最好能确定高危易损斑块的冠状动脉内测量与CT血管造影等无创成像之间的相关性。这项研究只针对那些因罪犯病变而来实验室的患者。问题是这些易损斑块特征也可能在没有罪犯病变的患者身上发现。对我来说,重要的是下一步要找到与这些血管内发现相关的非侵入性检查。

Erlinge希望有一天CT血管造影能够被用来识别那些有富含脂质的易损斑块的潜在高危患者,然后将其转诊以进行更明确的血管内成像。目前,如果在IVUS、NIRS或OCT上检测到易损斑块,Erlinge说道,医生在等待临床研究以确定这些病变的血运重建是否能降低临床事件的风险时,应该加倍使用积极的降脂疗法。
