首都医科大学附属北京同仁医院杨金奎教授团队于4月8日发表在medRxiv预印本上一项回顾性病例系列分析,揭示血糖监测可预测新冠肺炎患者的多脏器受损,并作为预测死亡风险的最早期指标。血糖监测简便易行,新冠肺炎患者测定血糖有利于病情判断,尽早发现重症患者并及时强化治疗。

 


研究背景

近来,新型冠状病毒(SARS-CoV-2)疾病(COVID-19)在全球大流行。COVID-19患者多数为轻症,中症患者通常在一周后出现呼吸困难,而重症患者可迅速发展到包括急性呼吸窘迫综合征(ARDS)、急性呼吸衰竭、多脏器受损、凝血障碍、脓毒性休克和代谢性酸中毒在内的危重症。及早识别危重症的危险因素有助于及时迅速将患者转入重症监护室(ICU),从而挽救更多的生命。本项研究通过分析多例中症、重症、危重症COVID-19患者的实验室检查结果,探索影响预测COVID-19死亡的关键因素。

研究方法

这项病例系列分析纳入了2020年1月29日~3月20日北京同仁医院医疗队在武汉协和医院救治的69例中重度COVID-19患者,已排除糖尿病、慢性心脏病、肾脏/肝脏疾病。研究分析资料包括基本人口信息,肌酸激酶(CK)、肌酸激酶同工酶(CK-MB)、天冬氨酸转氨酶(AST)、羟丁酸脱氢酶(HBDH)、碱性磷酸酶(AKP)、血尿素氮(BUN)、乳酸脱氢酶(LDH)、丙氨酸以转氨酶(ALT)、肌酐(Cr)、空腹血糖(FBG)等反映心脏、肝脏、肾脏、胰腺功能的指标。

研究结果:69例患者平均年龄61岁(四分位间距:52~67岁),年龄越大,病情越严重(P=0.024)。其中,23例中症、20例重症、26例危重症;53例幸存,16例死亡。34例(49.3%)男性,幸存者中男性21例(39.6%),死亡者中男性13例(81.2%),男性死亡率更高(RR=2.0,P=0.004,95%CI:1.28~3.02)。16例患者自发病至死亡时长14天(四分位间距10~22天),半数死亡患者在住院后3天左右死亡(四分位间距3-7天)。危重症患者及死亡患者中心肌损伤指标LDH、HBDH、CK、CK-MB显著升高;死亡患者中肾脏损伤指标BUN、Cr升高;未见肝脏损伤指标ALT、AST升高。

单变量Logistic回归风险模型显示,男性(HR=5.02,95%CI 1.43-17.62)、FBG升高(HR=5.09,95%CI 1.76-14.70)、LDH升高(HR=9.73,95%CI 1.28-73.7)和HBDH升高(HR=3.81,95%CI 1.23-11.85)均可预测死亡风险。

多变量Cox回归风险模型显示,FBG≥7mmol/L是唯一独立的死亡预测因子(HR=3.75,95%CI:1.26-11.15)。

聚类分析模型显示,入院时FBG与住院2~4天后LDH、HDDH、Cr更相关(r=0.43、0.43、0.50,P<0.01)。提示在死亡患者中,血糖升高比其它指标升高更早,预测价值更大。

研究结论

血糖升高是预测COVID患者多器官损伤的独立危险因子,可早期预测COVID患者的死亡风险。血糖监测简单易行,其结果有助于临床医生预测多器官损伤风险,进而将患者及时转入重症监护室并给予强化治疗。
