Thorax:血管供体单核细胞在肺移植缺血再灌注损伤中发挥关键作用!

这些结果表明,肺血管内单核细胞保留作为肺移植中一种“乘客”白细胞群,在移植相关的缺血再灌注损伤中发挥着关键作用。

在肺移植接受者原发性移植物功能障碍源于最初的、主要是白细胞依赖的缺血再灌注损伤。可见肺血管单核细胞已被证明在实验性急性肺损伤中起着关键作用,但对肺移植后缺血再灌注损伤的作用仍是未知的。近日,胸部疾病领域权威杂志Thorax上发表了一篇研究文章,研究人员旨在探讨供体血管内单核细胞在肺移植相关的急性肺损伤和原发性移植物功能障碍中的作用。

研究人员分离灌注的C57BL/6小鼠的肺脏,将其在正常氧分压条件下进行热缺血(2 小时)和再灌注(2 小时)。研究人员测定了单核细胞保留、活化表型,以及通过静脉注射氯膦酸二钠脂质体治疗耗竭它们对肺脏炎症和损伤的效应。在人类供体肺移植的样本中,研究人员通过流式细胞仪检测了单核细胞存在情况及其活化表型(低侧散射,27E10 +,CD14 +,HLA-DR,CCR2 +),并对移植后的肺功能进行了比较。

在缺血再灌注小鼠肺脏微血管中可见大量的单核细胞,降低L-选择素和增加CD86的表达表明其活化。单核细胞耗竭导致肺湿干比值、支气管肺泡灌洗液蛋白和灌流液的RAGE、MIP-2和KC水平降低,而补充单核细胞后可以导致局部损伤修复。在人的肺脏中,移植前供体单核细胞数/它们的CD86和TREM-1表达与移植后48和72 小时肺脏功能障碍相关。

这些结果表明,肺血管内单核细胞保留作为肺移植中一种“乘客”白细胞群,在移植相关的缺血再灌注损伤中发挥着关键作用。
