Neurology: 浙江大学:脊髓小脑共济失调,神经丝轻链水平与疾病严重程度息息相关

血清NfL水平与SCA2患者的疾病强度相关,并且在接近SCA2临床前患者的估计发病年龄时水平较高。

脊髓小脑共济失调2型(SCA2)是世界上最常见的常染色体显性遗传性共济失调之一,其临床特征是进行性共济失调、缓慢的低度囊性运动、反射不足和多发性神经病变。考虑到即将到来的疾病改良治疗和几种有希望的疗法,立即需要可获得的、可靠的和客观的工具,以对SCA2患者进行不同阶段的分层,评估疾病的严重程度,跟踪疾病的进展,并进一步为未来的试验铺平道路。虽然广泛的努力已经建立了神经影像学和电生理学的进展生物标志物,但很少有生物化学标志物被确定。


神经丝光(NfL)链作为一种候选生物标志物正得到越来越多的关注,因为NfL的释放在轴突损伤时急剧增加。最近的研究验证了NfL在许多神经退行性疾病中是一个有希望的生物标志物,包括帕金森病、亨廷顿氏病(HD)、肌萎缩性侧索硬化症(ALS)和痴呆症。在脊髓小脑共济失调3型(SCA3)患者的CSF和血清中NfL较高,并与临床严重程度相关。然而,以前的一项工作观察到两名SCA2患者的NfL水平没有明显升高。因此,迫切需要确定NfL是否可以作为更大范围内的神经变性的潜在生物标志物。

在这项研究中,浙江大学的吴志英等人,调查不同疾病阶段的SCA2患者的血清和脑脊液NfL水平,并确定NfL是否与疾病严重程度有关。

他们从中国的一个医疗中心招募参与者,并对SCA2患者进行基因诊断。使用单分子阵列方法评估NfL水平。使用共济失调评估量表(SARA)、国际合作共济失调评定量表(ICARS)和非共济失调症状清单(INAS)评估疾病严重程度。小脑和脑干的体积是用神经影像学的测量结果计算的。我们使用皮尔逊相关和部分相关进行相关分析。

49名SCA2患者、10名SCA2临床前患者和92名对照组被纳入。血清和脑脊液NfL之间有高度的一致性(r = 0.868, p < 0.0001)。在SCA2患者中,调整年龄后,血清NfL水平与疾病严重程度相关(SARA,r = 0.425,p = 0.003;ICARS,r = 0.383,p = 0.009;INAS,r = 0.390,p = 0.007;小脑体积,r = - 0.393,p = 0.024)。NfL水平在接近SCA2临床前个体的预期发病年龄时较高(R2 = 0.43,p = 0.04)。

血清NfL水平与SCA2患者的疾病强度相关,并且在接近SCA2临床前患者的估计发病年龄时水平较高。因此,NfL是SCA2疾病严重程度的一个潜在的血清生物标志物。
