Eur Urol:磁共振成像定位对前列腺癌过度诊断和过度治疗的影响

近期,有研究人员确定了良性系统活检结果的男性中,通过MRI却发现存在高等级癌症的死亡风险情况

研究人员使用了国家癌症研究所MRI研究中999名系统活检阴性并同时进行MRI靶向活检男性的数据。比较组包括欧洲前列腺癌筛查随机试验(ERSPC)中3056名六处定位活检阴性后随访11年的男性。研究人员计算了靶向活检后需要诊断(NND)和治疗(NNT)的患者人数。他们使用了一个简单的建模方法,其中涉及几个假设,如ERSPC的死亡比例可以通过MRI引导下活检的早期检测来预防。之后改变这些假设,以评估对结果的影响。

研究结果发现,在对MRI有利的假设的情况下,NND和NNT分别为89和57,而在比较中性的假设下,NND和NNT分别为169和127。在不太可能的情况下,如MRI的敏感性为100%,治疗的治愈率为100%,MRI定位的结果才会更令人鼓舞。
