JAHA:阻塞性睡眠呼吸暂停与房颤的因果关系

该MR分析发现,遗传预测的OSA与增加的房颤风险之间存在因果关系。

房颤是最常见的心律失常,影响1%至4%的人群,由于人口增长和老龄化,预计未来30年其患病率将在全球范围内增加。在观察性研究中,阻塞性睡眠呼吸暂停(OSA)已显示与房颤风险增加有关。这种关联是否反映了因果效应尚未明确。

近日,血管疾病领域权威杂志JAHA上发表了一篇研究文章,该研究的目的是评估OSA对心房颤动的因果影响。

研究人员使用2样本孟德尔随机化(MR)方法来评估OSA对心房颤动的因果影响。遗传变异与OSA关联的汇总数据来自最近发表的一项涉及多达217955名个体的全基因组关联研究,以及另一项涉及多达1030836名个体的全基因组关联研究的变异性房颤关联数据。研究人员使用逆方差加权方法评估效果估计值。在敏感性分析中进行了其他MR分析,包括惩罚逆方差加权、惩罚稳健逆方差加权、MR-Egger、简单中值、加权中值、基于加权模式的估计和孟德尔随机化多效残差和异常值方法。

使用5个单核苷酸多态性作为工具,在固定效应和随机效应逆方差加权模型中的MR分析表明,遗传预测的OSA与房颤风险增加相关(优势比[OR]为1.21;95%CI为1.12-1.31,P<0.001;OR为1.21;95%CI为1.11–1.32,P<0.001)。MR-Egger表明没有遗传多效性的证据(截距为-0.014;95%CI为-0.033至0.005,P=0.14)。在敏感性分析中使用其他MR方法的结果较为稳健。

由此可见,该MR分析发现,遗传预测的OSA与增加的房颤风险之间存在因果关系。
