Crit Care Med:评估ICU出院患者的预后—血清半胱氨酸蛋白酶抑制剂C与肌酐谁更强?

近期,一篇发表在杂志Crit Care Med上的文章进行了回顾性观察队列研究。在3,077名成人ICU幸存者中,研究者们比较了ICU患者中排出的半胱氨酸蛋白酶抑制剂C和肌酐及其与1年死亡率的关系。排除标准为,ICU出院72小时内死亡,ICU停留时间不超过24小时,终末期肾病患者。

在ICU入院期间,血清半胱氨酸蛋白酶抑制剂C和肌酐发生了偏离,与肌酐估算的肾小球滤过率相比,使用半胱氨酸蛋白酶抑制剂C估计的肾小球滤过率小于60 mL/min/1.73 m,44% vs. 26%。
在743例无急性肾损伤的患者中,ICU出院后的肾功能应反映当时的基线,通过肌酐估计的肾小球滤过率一直高于随访的肌酐估计的肾小球滤过率,而通过半胱氨酸蛋白酶抑制剂C估计的肾小球滤过率与随后的慢性肾脏疾病状态匹配良好。

至1年的时间,535名患者(17.4%)死亡。在对年龄、性别和合并症进行的生存分析中,半胱氨酸蛋白酶抑制剂C与死亡率增加呈线性相关,风险比等于1.78(95%CI,1.46-2.18)。相反,肌酐与死亡率呈现J型关系,因此在大多数患者中,与生存率无显着相关性,风险比等于1.03(0.87-1.2)。在调整肌酐和半胱氨酸蛋白酶抑制剂C水平后,较高的出院肌酐水平与长期死亡率较低有关。

此项研究表明:与肌酐相反,半胱氨酸蛋白酶抑制剂C与长期死亡率一致,能够识别高危和低危患者,与随访肾功能有更好的相关性。相反,相对于半胱氨酸蛋白酶抑制剂C而言,较低的肌酐显示出不利的预后。半胱氨酸蛋白酶抑制剂C值得进一步的研究,明确其是否可以作为严重疾病后评估肾功能的更有意义的测量指标。
