NEJM:PRIME细胞与类风湿性关节炎发作

对类风湿性关节炎患者进行基因组分析显示,耀斑爆发前,血液中存在的PRIME细胞通过B细胞激活后,从血液中迁移到滑膜,导致患者症状加重

类风湿性关节炎和许多炎症性疾病一样,存在静止和恶化(耀斑)期,但导致耀斑的机制尚不清楚。
 
近日研究人员建立了一种临床技术方案,参与研究的类风湿关节炎患者接受多次血液采集,进行纵向RNA测序。在4年的时间,对患者类风湿性关节炎耀斑期间多个时间点进行采样,比较耀斑前后滑膜单细胞样本RNA序列差异。
 
研究发现,在类风湿性关节炎发作前1至2周,血液转录谱出现变化:在B细胞活化之后,类风湿性关节炎患者血液循环中 CD45-CD31-PDPN+预炎性间充质细胞(也称PRIME细胞)扩增显著,这些细胞具有炎性滑膜成纤维细胞的共同特征,而耀斑期间,患者循环中PRIME细胞水平下降。流式细胞和分类细胞RNA-seq序列证实其他19名类风湿关节炎患者血液中存在PRIME细胞。
