CLIN CANCER RES:胰腺囊肿新的鉴别方法

约2-3%的成人存在胰腺囊肿。但是目前的诊断方法无法准确地区分良性囊肿与可能进展为肿瘤的囊肿。异常的细胞周围蛋白水解是恶性病情的标志。CLIN CANCER RES近期发表了一篇文章,研究蛋白酶活性是否可以区分良性的非粘液性囊肿与癌前病变粘液性囊肿。

约2-3%的成人存在胰腺囊肿。但是目前的诊断方法无法准确地区分良性囊肿与可能进展为肿瘤的囊肿。异常的细胞周围蛋白水解是恶性病情的标志。CLIN CANCER RES近期发表了一篇文章,研究蛋白酶活性是否可以区分良性的非粘液性囊肿与癌前病变粘液性囊肿。

作者对23例囊液标本进行了全蛋白酶谱检测确定蛋白酶活性以选取目标蛋白酶。通过110例样本分析确定了目前蛋白酶的鉴别活力,且需要的囊液量小于5uL。研究结果表明,天冬酰胺蛋白酶gastricsin和cathepsin E在粘液性囊肿的囊液中活性升高。免疫组化分析表明gastricsin表达与低级别不典型增生有关,cathepsin E表达与不典型增生无关。gastricsin活性区分粘液性囊肿与非粘液性囊肿的特异度为100%,灵敏度为93%。cathepsin E活性区分粘液性囊肿与非粘液性囊肿的特异度为92%,灵敏度为70%。gastricsin显着优于目前广泛使用的分子标志CEA,CEA的特异度为94%,灵敏度为65%。gastricsin联合CEA的诊断特异度为100%,灵敏度为98%。

文章最后认为,gastricsin和cathepsin E活性可以准确区分粘液性胰腺囊肿和非粘液性胰腺囊肿,具有取代目前诊断方式的潜力。
