简要介绍:

肋骨骨折是最常见的胸外伤。2000 年, 美国有 30 余万人因肋骨骨折就诊,其中约 18 万人需住院治疗。创伤性肋骨骨折的治疗主要分为保守治疗和手术治疗,对于连枷胸等重度和复杂(多根、高龄和特殊部位)肋骨骨折,虽然自上世纪五十年代以来呼吸机辅助呼吸内固定已被广泛接受为标准治疗,但近年来随着材料学的发展、手术技巧及麻醉技术的进步,手术复位内固定重新受到重视。大量数据也表明,手术复位内固定相较保守治疗更具优势。遗憾的是,由于许多急诊创伤科、重症医学科和胸外科医生对肋骨骨折治疗的最新理念了解不足,再加上目前尚缺乏高级别临床证据,导致手术在肋骨骨折治疗中的作用还存在诸多争议,也限制了该技术的规范开展。另一方面,在我国一些基层医院,对某些经保守治疗可取得良好效果的简单肋骨骨折也有进行手术的情况。因此,对肋骨骨折的外科治疗,过度治疗和治疗不足是普遍存在的现象。

为此,广东省医疗行业协会胸外科管理分会联合广东省医师协会胸外科分会采用国际通用的Delphi 程序,检索 Medline、The Cochrane Library 和万方数据库等,回顾性分析 1940 年 1 月至 2016 年12 月间关于创伤性肋骨骨折的文献,选取其中证据级别较高,研究内容契合临床实践的文章,制定存在争议的调查项目,就创伤性肋骨骨折诊疗中存在争议的问题进行调查(包括手术适应证、手术时机、手术方法等 18 个方面),对广东省内可以开展肋骨骨折内固定手术的 126 家医疗机构发出调查问卷,收回问卷 110 份,了解专家意见和临床实践的真实情况,并分别于 2016 年 12 月和 2017 年 3 月两次进行专家面对面的讨论,最后达成共识。
