ATVB:缺血性心脏病患者血管周围脂肪组织炎症的意义

源自功能失调的PVAT的促动脉粥样硬化介质可能是人类血管疾病的机制。此外,PVAT在肥胖状态下可能存在有害特性,而肥胖在缺血性心脏病的病理生理机制中起关键作用。

人们日益认识到,源自脂肪组织的促动脉粥样硬化介质可导致肥胖相关的血管疾病。近日,心血管领域权威杂志Arteriosclerosis, Thrombosis, and Vascular Biology上发表了一篇研究文章,研究人员旨在归纳与动脉粥样硬化易感性相关的血管周脂肪组织(PVAT)表型的区域差异。

研究人员评估了34名接受瓣膜、主动脉或冠状动脉搭桥手术的受试者(体重指数为32±6 kg/m2,年龄为59±11岁)胸部PVAT样本,并使用全基因组表达谱和定量聚合酶链反应分析进行转录组特征。研究人员确定了炎症反应明显的PVAT区域,该区域围绕着人主动脉根部,紧邻冠状动脉起始处并累及心外膜脂肪。

在进行冠状动脉旁路移植术的受试者中,与非动脉粥样硬化性左侧内乳动脉相比,主动脉根部周围的PVAT样品中300个基因显著上调(错误发现率Q<0.1)。编码与动脉粥样硬化相关蛋白的基因在主动脉PVAT中富集,且由与炎症、WNT信号传导、基质重塑、凝血和血管生成相关的信号传导途径组成。

定量聚合酶链反应证实了包括IL1β、CCL2(MCP-1)和IL6在内的几种促动脉粥样硬化转录产物高表达,并在冠心病患者中得到了证实。Gensini评分量化的血管造影冠状动脉疾病负担与PVAT中炎症基因的表达呈正相关。此外,与非肥胖者相比,IL1β表达显著上调(约8倍)的肥胖受试者腹主动脉脂肪炎症水平明显更高。

由此可见,源自功能失调的PVAT的促动脉粥样硬化介质可能是人类血管疾病的机制。此外,PVAT在肥胖状态下可能存在有害特性,而肥胖在缺血性心脏病的病理生理机制中起关键作用。
