Sci Transl Med:肾上腺素能系统为靶点抑制脑出血后神经炎症

大脑有一种自我保护机制,通过β3--肾上腺素能神经支配来调动独特的单核细胞群体,这些单核细胞直接指向受伤的大脑。

脑出血患者血液循环中髓细胞增多,外周免疫细胞浸润脑实质。单核细胞增加的机制及其对脑出血的影响仍有待阐明。Shi等人发现,在脑出血患者和小鼠模型中,骨髓造血干细胞(HSCs)增加。在小鼠中,肾上腺素能神经支配介导脑出血后造血功能的增加,新产生的细胞浸润大脑抑制神经炎症,减少脑损伤。增加肾上腺素能信号对小鼠有治疗作用。结果表明,以肾上腺素能系统为靶点可能是抑制脑出血后神经炎症的有效途径。该文发表在Science Translational Medicine杂志。

急性脑损伤动员白细胞迁移到血管周围空间和脑实质。这个过程会放大神经损伤。骨髓造血系统补充耗尽的外周血白细胞。然而,目前尚不清楚脑损伤是否影响骨髓谱系的发育,以及改变的造血细胞谱系如何影响神经结果。

从大脑和外周(淋巴结和脾脏)已经获得了许多关于神经免疫相互作用及其潜在临床相关性的知识。该研究提供了ICH诱导的急性脑损伤背景下脑与骨髓之间神经免疫相互作用的观点。这种见解是通过从ICH患者获取颅骨瓣来推断的。这种独特的方法使我们能够获得骨髓细胞,骨髓细胞经历了明显的谱系发育偏倚,特别是急性ICH期间骨髓细胞祖细胞的增加和淋巴细胞祖细胞的伴随减少。通过ICH患者和两种ICH小鼠模型的偶联实验进行的机制研究揭示了CNS和免疫系统之间的串扰作为高度协调的过程,由肾上腺素能神经支配控制。新生成的Ly6Clow巡视单核细胞从骨髓迁移到脑出血脑内,这些细胞抑制神经炎症并降低PHE,从而改善神经学结果。

该研究表明,在脑出血(ICH)患者和实验性脑出血模型中,骨髓造血干细胞(HSCs)可迅速向髓系谱系倾斜。谱系追踪显示,Ly6Clow单核细胞浸润脑出血大脑,造血功能增强,产生活化巨噬细胞,抑制神经炎症和脑损伤。ICH脑使用涉及细胞分裂周期42的β3-肾上腺素能神经支配来促进Ly6Clow单核细胞的骨髓造血,这可以通过美国食品和药物管理局批准的β3-肾上腺素能激动剂mirabegron进一步增强。脑损伤可调节星状细胞谱系发育以抑制远端脑炎症,暗示骨髓作为自我保护神经免疫相互作用的独特利基,可能被用于获得治疗效果。
