Neurology:生育史影响中年痴呆风险

更多雌激素暴露的生殖史事件,如绝经前状态、更长的生殖跨度、更多的儿童、使用HT和HC,与中年妇女更大的GMV相关

最近的研究发现,女性特有的生殖史事件可能是女性阿尔茨海默病(AD)更高患病率的生物学基础。雌激素神经保护作用的临床前证据也表明女性生育能力与AD之间存在联系。

Lisa Mosconi等研究女性生育史雌激素暴露指标与中年阿尔茨海默病(AD)脑MRI生物标志物之间的关系。

该研究利用生殖史数据、神经心理学测试和容积MRI扫描评估了99名52±6岁的认知正常女性和29名52±7岁的男性。使用多元回归来检验生殖史指标、体素灰质体积(GMV)、记忆和整体认知得分之间的相关性,并调整人口统计学和中年健康指标。暴露变量包括更年期状态、初潮年龄、更年期年龄、生育期、子宫切除术状态、儿童和怀孕人数、更年期激素治疗(HT)和激素避孕药(HC)的使用。

与男性相比,所有更年期组AD易感区域的GMV均较低,围绝经期组和绝经后组颞叶皮质的GMV也较绝经前组低。

生殖跨度、儿童和怀孕数量、HT和HC的使用与GMV呈正相关,主要在颞叶皮质、额叶皮质和楔前叶,与年龄、APOE-4状态和中年健康指标无关。尽管生殖史指标与认知测量没有直接关系,但颞区GMV与记忆和整体认知得分呈正相关。

该研究表明更多雌激素暴露的生殖史事件,如绝经前状态、更长的生殖跨度、更多的儿童、使用HT和HC,与中年妇女更大的GMV相关。需要进一步的研究来阐明生殖史影响认知老化和AD风险的性别特异性生物学途径。
