NEJM:新冠肺炎血清抗体状态与二次感染风险研究

对于新冠肺炎血清学抗体阳性的人群,其随后6个月内二次感染的风险较低

严重急性呼吸综合征冠状病毒2(SARS-Cov-2)感染后,大多数病例可产生免疫应答反应,但抗体保护的效果及持续时间尚不清楚。近日研究人员对英国牛津大学医院的医疗工作人员进行随访,考察了新冠肺炎血清学阳性或血清学阴性,与有新冠肺炎症状或无症状感染和经聚合酶链反应(PCR)证实的SARS-cov-2二次感染之间的关系。
 
医护人员基线的血清学抗体情况通过Spike蛋白抗体和IgG抗体进行评估,随访期为31周。根据抗体状况,年龄、性别和发病率随时间的变化,评估PCR阳性检测结果和症状性感染的相对发生率。
 
12541名健康的医护人员接受抗体检测,其中阴性11364人,阳性1265人,而88人随访期间出现血清转化。随访期间,223名Spike蛋白抗体阴性的医护人员经PCR证实核酸阳性(每10000参与者随访天中1.09例),其中100人是无症状的,123人有新冠肺炎症状,仅2名Spike蛋白抗体阳性的医护人员经PCR证实核酸阳性(每10000参与者随访天中0.13例),均是有症状的患者(症状性感染风险比值比0.11)。Spike蛋白抗体阳性的医护人员中,未发现无症状感染者。基线采用Spike蛋白抗体、IgG抗体或二者联合使用进行血清学评估不影响研究结论。
抗体状态与二次感染风险
研究认为,对于新冠肺炎血清学抗体阳性的人群,其随后6个月内二次感染的风险较低。
