新研究:类黄酮摄入与亚临床动脉粥样硬化之间的关系

在这个多民族人群中,较高的饮食类黄酮摄入量与较低的外周动脉和颈动脉粥样硬化的几率相关。增加摄入健康的富含类黄酮的食物可以防止外周动脉和颈动脉粥样硬化。

黄酮类化合物可能在减轻动脉粥样硬化性心血管疾病中发挥作用,有证据表明不同血管床的效果可能不同。不同潜在动脉粥样硬化风险的亚人群之间动脉粥样硬化亚临床标志物的相关性研究尚缺乏。
 
在多种族动脉粥样硬化研究(MESA)的5599名参与者中,研究人员使用重复测量模型评估了饮食类黄酮摄入量(从食物频率问卷中估计)与动脉粥样硬化亚临床测量(踝-肱指数,颈动脉斑块和内膜-中膜厚度,冠状动脉钙化)之间的关系。在评估1(2000-2002年)和评估5(2010-2011年)测量暴露因素和结局。此外,研究人员使用分层分析和相互作用术语来探讨时间、性别、种族/民族和吸烟状况对效果的影响。
 
在分析人群中,基线时约46%为男性,中位年龄为62岁(四分位数范围为53-70),总黄酮摄入量为182(四分位数范围为98-308)mg/d。在多变量调整后,在评估1和5的平均值中,总黄酮摄入量最高(四分位数4)和最低(四分位数1)的参与者踝关节-肱指数<1的几率低26%(比值比为0.74 [95%CI为0.60-0.92]),颈动脉斑块的几率低18%(比值比为0.82 [95%CI为0.69-0.99])。中度(四分位数3)至高(四分位数4)摄入黄酮醇、黄烷醇单体和花青素与踝关节-肱指数<1的几率降低19%至34%以及颈动脉斑块的几率降低18%至20%相关。在基线时,花青素摄入量最高的参与者(四分位数4)的颈动脉斑块进展速度略慢于摄入量中等的参与者(四分位数2和3)。而,花青素摄入量与内膜-中膜厚度或冠状动脉钙化没有显著关联。研究人员观察到的关联没有因性别、种族/民族或吸烟状况而异。
 
由此可见,在这个多民族人群中,较高的饮食类黄酮摄入量与较低的外周动脉和颈动脉粥样硬化的几率相关。增加摄入健康的富含类黄酮的食物可以防止外周动脉和颈动脉粥样硬化。
