背景:研究已经证实,多不饱和脂肪酸(PUFA)和肥胖之间存在相关性。妊娠期中的PUFA对于产后体重保留并促成长期肥胖是否具有影响尚未可知。

目的:旨在检测亚洲多民族队列中,产妇妊娠期中的血浆PUFA与产后18个月体重保留(PPWR)之间的关系。

设计:研究人员纳入了2009年6月~2010年9月前瞻性队列中的孕妇(653人)。怀胎26~28周时,测量了血浆磷脂酰胆碱PUFA浓度,并确定了其占总脂肪酸(FA)的比例。PPWR基于首次产科门诊就诊时测量的体重和产后18个月测量的体重间的差异计算。

结果:产后18个月时的中位保留体重为0.90kg(IQR,-1.40~3.25)。653名女性中,544人(83.3%)的PPWR<5kg,109人(16.7%)的PPWR≥5kg。调整线性回归模型中,较高的血浆二十碳五烯酸(EPA)、二十二碳六烯酸(DHA)、总ω-3(n-3)PUFA浓度与较低的PPWR相关[EPA:β=-0.62kg/总FA每增加1%(95% CI,-1.18~-0.05);DHA:β=-0.24kg/总FA每增加1%(95% CI,-0.45~-0.02);总ω-3 PUFA:β=-0.20kg/总FA每增加1%(95% CI,-0.36~-0.03)],而较高的血浆ω-6与ω-3 PUFA比值与较高的PPWR相关[β=0.21kg/增加1单位(95% CI,0.05~0.36)]。

结论:妊娠晚期较高的血浆ω-3 PUFA百分比和较低的ω-6与ω-3 PUFA比值与产后18个月时较低的体重保留相关。通过妊娠期的膳食或鱼油补充来增加EPA和DHA状态,降低ω-6与ω-3 PUFA比值,可为协助产后减重提供一种替代策略。
