研究人员寻求一种新方法来验证肿瘤接触表面积作为肾细胞癌部分肾切除患者术后肾功能的预测因子。

研究人员对2014年至2016年进行部分肾切除术的患者进行了前瞻性的肾癌数据库的查询。
利用术前横截面成像获得的数据计算接触面积。研究了接触表面积与围手术期变量之间的关系。评估术后肾功能结果、接触表面积和r.e.n.a.l(桡骨、外生/内生特性、肿瘤与采集系统或窦的近距离、前/后、与极线及肿瘤的位置相对于极线及肿瘤接触的主要肾动脉或静脉)的相关性。

共有257例接受部分肾切除术的患者有足够的数据进入研究。中位接触面面积为14.5 cm2 (IQR 6.2-36),中位肾功能评分为9 (IQR 7-10)。斯皮尔曼相关分析显示,接触表面积与估计失血(rs = 0.42, p <0.001),停留时间(rs = 0.18, p = 0.005),以及估计肾小球滤过率的百分比和绝对变化有关(rs = -0.77和-0.78,分别为p <0.001)。在多变量分析中,接触表面积和肾测量分数是预测肾小球滤过率绝对变化的独立预测因子(每p <0.001)。ROC曲线分析显示,与肾小球滤过率(AUC 0.94 vs 0.80)相比,接触表面积是预测肾小球滤过率大于20%的较好预测指标。

结论
部分肾切除术后,切面面积与术后肾功能变化相关。它可与肾脏病评分结合使用,为患者预测部分肾切除术后肾功能减退的风险。
