JAHA:乙酰水杨酸与升主动脉瘤发病率降低和主动脉环氧合酶2表达降低相关

ASA治疗可能通过抑制升主动脉壁中的环氧合酶-2和随后的抗炎作用来延缓升主动脉瘤的进展。

升主动脉瘤 (AscAA) 是一种多原因导致的升主动脉进行性可能危及生命的疾病。内侧退行性变合并低度炎症是一些但不是所有类型AscAA的共同特征。乙酰水杨酸 (ASA) 治疗与降低腹部和颅内动脉瘤的患病率和生长速度有关,但ASA与升主动脉瘤形成之间的关系仍然很大程度上未知。

近日,心血管疾病领域权威杂志JAHA上发表了一篇研究文章,该研究目的是调查ASA治疗是否与手术队列中较低的升主动脉瘤患病率之间的相关性。

在这项回顾性横断面研究中,研究人员对1700名因升主动脉瘤和/或主动脉瓣疾病接受心脏手术的患者进行了研究。主动脉扩张定义为主动脉根部或升主动脉直径≥45 mm。服用药物情况由患者在系统问卷中自我报告。研究人员在受试者升主动脉的内膜中层部分测量了环氧合酶基因表达(n=117)。

在这项多变量分析中,ASA与三尖瓣主动脉瓣患者的升主动脉瘤患病率降低相关(相对风险为0.68 [95%CI为0.48-0.95],P=0.026),但与二尖瓣主动脉瓣患者无关(相对风险为0.93 [95%CI为0.64-1.34],P=0.687)。内膜中层环氧合酶表达与升主动脉尺寸呈正相关(环氧合酶-1 P<0.001,环氧合酶-2 P=0.05)。在扩张AscAA但非扩张的三尖瓣主动脉标本中,ASA与显著降低的环氧合酶-2水平相关(P=0.034)。

由此可见,ASA治疗可能通过抑制升主动脉壁中的环氧合酶-2和随后的抗炎作用来延缓升主动脉瘤的进展。
